Job Description

Position Summary

Under general supervision, the Program Coordinator Assistant, is responsible for supporting various functions of the NKFM’s public health education programs. This position is responsible for stewarding the initial participant inquiry/outreach process. The secondary responsibility of this position is to ensure supply management and administrative functions are timely and efficient. The Program Coordinator Assistant is expected to work closely with other NKFM Program Coordinators to ensure participants, program cohorts, special events and/or promotional events are fully supported.
Major Responsibilities

The responsibilities of the Program Coordinator Assistant will include, but are not limited to, the following:

Program Support and Organization
● Manage incoming participant referrals for NKFM programs
● Demonstrate empathy and compassion as the first contact for potential participants
● Receive, organizes, and assigns all incoming inquiries via online portals, secure fax, phone call, and email.
● Provide warm introduction to the Diabetes Prevention or other programs to participants who are referred by a health care provider, have been identified by a health plan, or have requested more information about a program
● Track all referrals and inquiries in the referral application
● Complete 100+ outreach phone calls per week
● Provide phone and email support to potential participants
● Develop and implement process(es) for program workshop materials distribution
● Troubleshoots logistical challenges and implements solutions
Cohort Support
● Collaborates with Coordinators and Program Leads to execute participant outreach activities as needed per workshop
● Supports others with the team problem solving process. Prompts others to contribute knowledge and information to assist the team. Encourages the team to remain focused and on track and resolves any interpersonal issues arising that may jeopardize team success.

Other Responsibilities
● Communicate effectively with team members, supervisors, and community members
● Always represent the NKFM appropriately in a professional manner
Position Qualifications
Education and Experience
● High School diploma required
● Some administrative or phone outreach experience preferred
● Experience in a community-based, or public facing role preferred
● Ability to demonstrate proficiency in MS Office Suite computer programs

Competencies
● Basic knowledge of or willingness to learn the Diabetes Prevention Program, Chronic Disease Self-Management Education, or similar programs
● Strong analytical and problem-solving skills
● Excellent communication and presentation skills
● Ability to work independently and as part of a team
● Attention to detail and accuracy
● Effective communication with community partners, stakeholders, and participants
● Communicate effectively with team members, supervisors, and community members
● Ability to communicate respectfully and effectively with diverse groups
● Verbal and written communication skills

Working Conditions
● Office environment with occasional requirements to attend off-site meetings or events
● Hybrid (remote, in person) optional.
● Ability to work under pressure and meet deadlines
● Some positions may require occasional travel

Physical Requirements
● Ability to sit for extended periods
● Ability to lift and carry up to 25 pounds
