Description: Case Manager – Homeless Outreach

As a Case Manager, you'll work directly with adults experiencing chronic homelessness who have co-occurring serious mental illness (SMI) and substance use disorders. You'll provide intensive, community-based services that help individuals secure housing, improve daily functioning, manage mental health symptoms, and achieve greater independence.

What You'll Do

As a Case Manager, you will:

  • Build trusting, respectful relationships with clients while fostering hope and supporting recovery.
  • Conduct outreach in locations where individuals experiencing homelessness are commonly found, including shelters, day centers, food kitchens, the Frank Williams Housing Resource Center, and other community partner agencies.
  • Help clients obtain and maintain safe, affordable, and stable housing.
  • Teach and reinforce independent living skills, including budgeting, personal care, medication management, communication, and other daily living activities.
  • Support clients in overcoming barriers to recovery through individualized, strengths-based service planning.
  • Utilize Motivational Interviewing and other evidence-based practices to engage clients in services.
  • Coordinate care and connect clients with community resources such as peer support, outpatient treatment, vocational services, healthcare providers, and other supportive programs.
  • Assist with crisis prevention and intervention when needed.
  • Educate and collaborate with family members and natural supports when appropriate.
  • Provide transportation and accompany clients to appointments, housing meetings, and community activities as medically necessary.
  • Complete timely, accurate documentation in accordance with agency and regulatory standards.
  • Collaborate with an interdisciplinary team to ensure high-quality, person-centered care.
Requirements: What We're Looking For
Required Qualifications
  • Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or a related field; or
  • Four years of equivalent experience in a community mental health setting.
Preferred Knowledge & Skills
  • Experience working with individuals experiencing homelessness, serious mental illness, and/or substance use disorders.
  • Knowledge of recovery-oriented care, trauma-informed practices, Motivational Interviewing, and evidence-based behavioral health interventions.
  • Understanding of community mental health systems, case management practices, and applicable laws and regulations.
  • Ability to build strong partnerships with community organizations and service providers.
  • Excellent communication, organization, and problem-solving skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Professional judgment and emotional resilience when responding to challenging behaviors or crisis situations.
