Job Description
Are you passionate about helping individuals successfully transition from treatment to the community and ensuring they have the support they need to thrive after discharge?
We're seeking an Aftercare Specialist to coordinate discharge planning, facilitate continuity of care, and build strong partnerships with community providers, courts, case managers, and treatment teams. In this role, you'll help remove barriers to successful community reintegration while serving as an advocate for individuals throughout the discharge and aftercare process.
What You'll Do
- Coordinate all aspects of the discharge process, including communication with aftercare providers, community agencies, case managers, forensic teams, and other stakeholders.
- Partner with social workers and the multidisciplinary treatment team to develop individualized discharge and conditional release plans.
- Identify barriers to successful discharge and implement strategies to promote safe and timely community reintegration.
- Build and maintain collaborative relationships with families, guardians, community mental health providers, court personnel, attorneys, forensic coordinators, and other external partners.
- Assess community placement and aftercare facilities to determine appropriateness and compliance with court or contractual requirements, when applicable.
- Accompany individuals served to court proceedings and assist with court-related discharge coordination as needed.
- When permitted by contract, maintain contact with individuals following discharge to support continuity of care, successful adjustment, and problem-solving during community reintegration.
- Maintain accurate, timely, and compliant documentation, medical records, discharge plans, and tracking lists.
- Participate in performance improvement initiatives that enhance discharge planning, care coordination, and service delivery.
- Demonstrate cultural competency, professionalism, and person-centered care in every interaction.
What We're Looking For
Education
- High school diploma or equivalent (required)
- Bachelor's degree in Social Work, Criminal Justice, Mental Health Counseling, or a related field (preferred)
Experience
- Two (2) years of experience working with individuals with severe and persistent mental illness (preferred)
- Three (3) years of experience working with courts, forensic programs, or related judicial systems (preferred)
