Job Description
MINIMUM QUALIFICATION
Education, Knowledge, Skills and Abilities
B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D) required.
Experience with hospital pharmacy computer systems preferred. Experience with Windows-based computers and software preferred.
Ability to effectively communicate with other practitioners as well as patients.
Strong customer service and team orientation.
Ability to accurately perform multiple tasks simultaneously and meet multiple demands.
Flexibility in day-to-day actions and duties.
Experience with current pharmacy technology, including Pyxis, automated compounding devices and robotics preferred.
Required Length and Type of Experience
Minimum 2 years of clinical pharmacy experience in ambulatory care, chronic disease management, anticoagulation services, infusion services, or related practice areas.
Required Licensure or Certification
Active license to practice pharmacy in The State of Ohio
Preferred Certifications
Board Certified Ambulatory Care Pharmacist (BCACP)
Board Certified Pharmacotherapy Specialist (BCPS)
Anticoagulation certification or related specialty training
Immunization certification
Required Physical and Environmental Demands
Dexterity sufficient to perform medication preparation and compounding functions.
Ability to lift materials weighing up to 25 pounds.
Ability to perform duties while walking, standing, stooping, bending and lifting.
GENERAL SUMMARY
The Chronic Care Pharmacist is responsible for developing, coordinating, and managing pharmacy services focused on chronic disease management, outpatient infusion therapy, and anticoagulation management. Pharmacists provide clinical services to optimize the effectiveness of patients’ medication therapy and improve patient outcomes. This role functions as an integral member of the interdisciplinary healthcare team, working collaboratively with physicians, advanced practice providers, nurses, case managers, and other healthcare professionals to optimize medication-related outcomes, provide drug information, improve quality metrics, enhance patient satisfaction, and reduce preventable hospitalizations. Pharmacists also supervise the technical staff in all areas of pharmacy practice.
