Job Description
Genesis Homecare Agency is seeking a motivated, compassionate, and community-focused professional to join our team as a Community Outreach & Client Care Coordinator.
This hybrid field position combines community marketing, client relationship development, client onboarding, and direct-care support. The ideal candidate will be comfortable representing Genesis Homecare Agency in the community while also providing hands-on caregiver support when coverage is needed.
Compensation
- $45,000 annual base salary
- Commission opportunities for eligible new client enrollments
- Full-time position
- Local travel required
Primary Responsibilities
Community Outreach and Marketing
- Develop relationships with senior communities, healthcare providers, rehabilitation facilities, social workers, case managers, churches, and other referral partners.
- Attend networking events, health fairs, senior events, community meetings, and marketing activities.
- Identify and develop new client referral opportunities.
- Conduct scheduled visits to prospective referral partners.
- Follow up with prospective clients, families, and community contacts.
- Distribute approved marketing materials and professionally represent Genesis Homecare Agency.
- Maintain accurate records of outreach visits, referrals, follow-ups, and new client leads.
- Assist with meeting monthly outreach and client-growth goals.
Client Intake and Field Coordination
- Conduct nonclinical in-home client assessments and intake visits.
- Explain available services, agency expectations, scheduling procedures, and client responsibilities.
- Conduct caregiver and client meet-and-greets before services begin.
- Help ensure caregivers understand the client’s service plan, schedule, routines, preferences, and authorized duties.
- Complete supervisor handoffs and communicate important information to the office team.
- Follow up with clients and families after services begin to ensure a smooth transition.
- Complete client satisfaction visits and address concerns appropriately.
- Document all assessments, visits, follow-ups, and client interactions accurately and promptly.
- Coordinate with a registered nurse or supervisor whenever a clinical assessment or higher-level review is required.
Caregiver and Client Support
- Serve as a fill-in caregiver when scheduled caregivers call out, coverage is unavailable, or a new case is beginning.
- Provide authorized assistance with activities of daily living and instrumental activities of daily living.
- Follow the client’s service plan and Genesis Homecare Agency policies.
- Accurately clock in and out through AxisCare and complete all required visit documentation.
- Report client concerns, incidents, schedule changes, and changes in condition promptly.
- Help stabilize new client cases while a permanent caregiver is being assigned.
Qualifications
- Previous experience in home care, senior care, healthcare marketing, community outreach, sales, or client relations is strongly preferred.
- CNA, PCA, or caregiver experience is preferred.
- Must meet all Genesis Homecare Agency caregiver onboarding requirements.
- Current CPR and First Aid certification or willingness to obtain certification.
- Successful completion of required background checks and health screenings.
- Valid driver’s license, reliable transportation, and current automobile insurance.
- Ability to travel throughout the assigned service area.
- Strong communication, relationship-building, organization, and follow-up skills.
- Comfortable speaking with clients, families, caregivers, healthcare professionals, and community partners.
- Ability to work independently and manage a flexible field schedule.
- Ability to provide direct personal-care assistance when needed.
- Proficiency with smartphones, email, electronic documentation, and scheduling systems; AxisCare experience is a plus.
- MUST be goal oriented and meet monthly quota.
Schedule and Work Environment
This is a full-time field-based position. The schedule will generally occur during normal business hours; however, occasional evenings or weekends may be required for community events, client meet-and-greets, urgent caregiver coverage, or other operational needs.
