How you will contribute:

The Sales Clinical Liaison acts as a patient advocate throughout the referral and admission process, ensuring patients are guided to the most appropriate level of care. This role requires a Registered Nurse (RN) or an individual with an advanced clinical degree.

How you will make an impact:

Business Development & Outreach: Proactively grow the organization by identifying new opportunities, building strong referral relationships, and representing the company in the community.

Patient & Family Engagement: Act as a trusted, compassionate resource, educating patients and families about hospice services and guiding them through enrollment.

Communication & Coordination: Strengthen partnerships with healthcare providers, ensure smooth communication between referral sources and clinical teams, and manage timely follow-ups and documentation.

Market Leadership: Drive awareness and growth by educating stakeholders, analyzing referral trends, staying informed on market dynamics, and positioning the organization as a trusted leader in the community.

The Experience and Skills You’ll Bring

  • Strong verbal and written communication skills with ability to engage clinical and non-clinical audiences
  • Effective relationship-building skills with referral sources, patients, and interdisciplinary teams
  • Customer service–oriented with strong active listening and professionalism
  • Ability to educate and influence using a consultative, clinically informed approach
  • Working knowledge of medical terminology, healthcare delivery systems, and referral processes
  • Familiarity with electronic health records (EHR); hospice and palliative care knowledge preferred
  • Strong organizational and time management skills with ability to manage multiple priorities
  • Proficiency in Microsoft Office and ability to learn new systems quickly
  • Demonstrates initiative, adaptability, and ability to work collaboratively in a team environment
  • Associate’s degree in nursing (ADN) or Bachelor of Science in Nursing (BSN)
  • Active, unrestricted Registered Nurse (RN) license in the state of practice.

Preferred Knowledge, Skills and Experience:

  • Knowledge of healthcare referral processes and case management
  • Experience with hospice care and understanding of prognostic indicators
  • Two (2) years of clinical nursing experience, including case management in a clinical setting
