The RN Case Manager will provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life. The RN Case Manager is typically part of a care team. This team collaborates to ensure individuals, and their families have the physical, emotional, and spiritual support they require throughout the care process.
