About This Position
Quality Assurance – RN Case Manager – Full Time -Day Shift- 7a-3:30- (Req 7936)
Pay Range $37.63-$49.86*
To assess and coordinate hospital services and resources deemed necessary to cost-effectively meet the patient’s needs, provide quality patient care and assure compliance with regulatory agencies from the time of patient’s admission through discharge.
Qualifications:

Graduate from an accredited school of nursing and has current New York State License as an RN (BSN is preferred).

2 years in community health field for discharge planning preferred.

Case Manager Certification preferred.

Certification to complete New York State PRI forms and assessment, interview, and coordination skills preferred.

Current knowledge and ability to apply NYDOH, TJC, CMS, and IPRO regulations to all areas of work.

Ability to interact and communicate effectively, verbally and in writing, with patients of various backgrounds and ages, and with hospital staff at all levels, especially physicians, department managers and nursing staff.

Ability to collect and analyze data, problem solve at all levels, and initiate or adapt internal systems to external regulations.

Ability to plan and conduct interdepartmental/interagency meetings.

Ability to handle stressful situations and adapt to changing tasks.
