Job Description
JOB SUMMARY
Responsible for solely providing sound diabetes care and education to a wide variety of patient populations. The specialist assesses, plans, applies and evaluates medical nutrition therapy for individuals within the program and participates in program functions and activities. The specialist serves as an integral part of the interprofessional team that providers collaborative, comprehensive and person-centered care, as well as education conducive to behavior change and improved quality of life across the lifespan. Participates in planning, presenting and evaluating health promotion community events and programs.
Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. Recognizes and demonstrates understanding of patient and family centered care.
