Job Description
POSITION SUMMARY: Assesses, plans, and coordinates resources/ services and provides support to patients and families at Gibson Cancer Center. Maintain a clear understanding of Oncology Policies and Procedures, as well as regulatory requirements and guidelines as it pertains to Oncology Accreditation. Provide psychosocial, financial, and emotional support to patients as needed to cope with his/her chronic or terminal illness. Provide early identification of needs and timely interventions as needed for the patient. Ensures proper documentation, collaboration with multi-disciplinary team members, identification of discharge plan and development of individualized care plan for patient. Monitor and follow through with patient’s treatment plan to assist them with meeting their goals.
EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE:
Minimum Required: Master’s Degree in Social Work with a minimum of 2 years of experience as medical social worker in Oncology, Illness, and/or Discharge Planning.
