Job Description
Case Manager
Under the direction of the Manager of Case Management, the Case Manager concurrently facilitates proactive patient care services to ensure that the care is appropriate, timely, cost-effective, and achieves the desired outcome. The Case Manager coordinates the care and service of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health case resources; and guides all disciplines toward postive quality outcomes. Qualifications: 1. Graduate of an accredited school of nursing, RN, BSN preferred, CM or CPHQ certification preferred. 2. 3 to 5 years of clinical experience, preferably Critical Care or General Med/Surg. 3. Demonstrated leadership ability. 4. Minimum 2 years in case management preferred. 5. Possesses effective written and verbal communication skills. 6. Current licensure by the Louisiana State Board of Nursing is required.
