About This Position

Description:

Functions and Scope of Job: This position is responsible for working with the Residential Supervisor to lead the team of Direct Support Professionals and maintaining the health and safety of the residents assigned to the home.

  • Act as Shift Supervisor; directly supervise team members assigned to the residence.
  • Provide supervision of all individuals while under his/her care.
  • Work as a member of a team to maintain a clean, safe, positive living environment for each individual who lives at the residence.
  • Monitor staff performance and conduct weekly HUGS, provide feedback, and evaluates the performance of Direct Support Professionals on the team.
  • Foster team building and positive productive work relationships at assigned residential homes. Understanding and training and supporting the Direct Support Professional to proficiency in Core Competencies.
  • Assist the Residential Supervisor to provide staffing support in the absence of a DSP by splitting the shift amongst the house team, utilizing the prn pool, or rotating shifts.
  • Monitor and ensure that all individuals receive their prescribed medication, and that the medication administration record has been completed correctly each day.
  • Ensure all residential documentation including logs and Incident/Accident (IA) reports are completed.
  • Plan and implement community activities for individuals in the residence.
  • Complete a draft of the monthly house staffing schedule to be submitted to the Residential Supervisor for review and final approval by the 20th of each month for the upcoming month.
  • Complete appointment summaries in THERAP documenting the results of any appointment attended to included follow-up, lab work, referrals.
  • Complete a grocery inventory in the home, and place bi-weekly grocery order to be submitted to the Residential Supervisor.
  • Complete supply inventory in the home, and place bi-weekly supply order to be submitted to Residential Supervisor.
  • Provide input of a draft of the Person Centered Review (PCR) quarterly and Person Centered Plans (PCPs) for residents in the home to include appointments, medication changes, behavioral or medical updates.
  • Complete Monthly Fire Drills, First Aid Kits (Home, Vehicle) for assigned home.
  • Complete weekly vehicle checks/ fuel vehicles on assigned day.
  • Complete inventory of individual clothing on a seasonal basis. Submit a request to purchase new clothing based on individual’s needs.
  • Other duties as assigned
Requirements:

RequirementsKnowledge, Skills, and Abilities and/or Competencies required to successfully perform the work: Must have a passion for and desire to work with people with intellectual disabilities and other physical or mental illnesses. Ability to demonstrate supervisory and administrative skills. Must have sufficient computer skills and excellent writing skills to complete documentation. Knowledge of and ability to perform simple nursing care, first-aid, behavioral management, and personal hygiene. Ability to observe, record and report participant’s behavior, attitude, and physical condition. Ability to perform basic math and communicate effectively, as well as excellent verbal and written communication skills. Ability to effectively train program staff and provide on-going support to encourage professional development of all staff. No convictions for crimes that preclude a person from working with members of a vulnerable population.

Minimum Requirements: Minimum of Bachelor’s degree in human services field preferred. At least two years of experience in a direct-care facility, preferably in a residential setting with individuals with an Intellectual or Developmental Disability/Mental Illness; education may be substituted for one year of experience. A recent criminal check, including a finger prints check. Valid VA driver's license with acceptable driving record. Clean Criminal records check. TB Screening. Ability to obtain First Aid/CPR certification.
