Position Description:

The Patient Navigator is a key member of Crouse Medical Practice who will provide a single and centralized point of patient access to assist clinicians and patients in managing specialty and diagnostic referrals. When necessary, the Patient Navigator will lead patients through the health care system from the initiation of the referral, to obtaining prior authorization, scheduling appointments and tracking and following up on results.

Required Qualifications: Registered Nurse, Demonstrated experience in a multi-specialty physician office. Excellent customer service skills. Excellent computer skills, Demonstrated competence interpersonal and intra professional relations. Demonstrated competence in acute care patient management/organization. Demonstrated commitment to collaboration with physician to individualize and enhance patient care.

Pay Range: $62,353.20 - $79,040.00 DOE
