Requirements of the Home Health LPN – Licensed Practical Nurse

Home Care experience preferred but not required. Looking for an experienced Home Health License Practical Nurse (LPN) to fill an opening in Glendale and surrounding areas. Responsibilities of the Home Health LPN

· Effectively collaborate with care team

· Patient assessment and evaluation of patients progress towards goals

· Implementation of care plan

· Plan and facilitate appropriate discharge

· Accurate and complete patient documentation

· Provide care which promotes quality, continuity, and patient safety

· Provide skilled nursing care to patients in the community

· Compliance with Electronic Visit Verification (EVV)

Requirements of the Home Health LPN

· Home Care experience preferred but not required

· LPN State License required

· Basic Life Support (BLS)

· Valid Drivers License

· Current Finger Print Clearance card

· 1 year or current TB test

· Reliable mode of transportation

· Malpractice Insurance
