CM supervisor also links with other community resources to ensure adequate provision of services to CMH clients. How you will make an impact:Provide clinical oversight and guidance to CM/Support Coordinators and Assistants regarding services to clients, including evaluation and recommendations regarding evidence based and promising practices.Participate in regularly scheduled supervision sessions with assigned employees, providing accurate performance feedback, and when necessary, implementing and monitoring disciplinary action.Assure assigned employees maintain a minimum compliance with all performance standards. Take immediate corrective action when standards are not met.Supervise staff in their provision of key areas of case management: assessment, planning, linking, coordinating, and monitoring of treatment to individuals diagnosed with severe mental illness, and/or developmental disability.Train, monitor and assure CM/Support Coordinators competence in Person-Centered Planning activities for I/DD Adult clients, including arranging for necessary assessments, compiling data, and designing the plan of service, crisis plan and/or Advance Directives as applicable.Maintain a case load of up to 10 case management recipients.Assure adherence to CMH eligibility criteria and level of care recommendations. Assure staff complete all Medicaid, MDHHS and/or other benefits documentation in a timely fashion.Provide leadership/development/implementation of waiver services to CMs.Ensure that the health and wellness needs of clients are addressed and integrated into treatment, via education and coordination with medical care providers.Assure CMs consult with staff psychiatrist and psychiatric nurse regarding medical and psychiatric interventions (e.g., medical compliance and reviews, injections, health care needs, etc.), including attending medical appointments with their clients.Assure CM/Support Coordinators liaison with contracted providers who are serving clients with intellectual/developmental disabilities. Assures CMS regularly conduct home visits, consultations, and reviews of individualized treatment plans/records.Act as liaison to various community stakeholders and partners including the local courts, law enforcement, ISD and local school systems, FQHC, guardians, Center on Aging, local MDHHS, Adult and Children’s Protective Services, contracted providers, etc. this may include representation on various committees.Participate in regional (SWMBH) meetings and workgroups as requested by direct supervisor.Act as liaison between CMHSP and designated inpatient care facilities to promote appropriate discharge planning.Assist in the identification of unmet needs and recommends improvements in services to clients and the community.Provide information and other public relations activities as requested.Attend clinical, agency and other related meetings as scheduled.Adhere to all policies and procedures of CMH, MDHHS, CARF, Medicaid, and other regulating bodies as may be applicable.Perform other duties and/or responsibilities as assigned. Minimum Education & Experience Requirements:MA/MSW degree in human services area with appropriate Licensure Valid driver’s licensePreferred QualificationsMinimum of 4 years of post-graduate work experience working with the mentally ill, co-occurring disorders or intellectual disabilitiesMinimum of 2 years of supervisory experienceAccess to reliable transportationAny combination of education, training and experience providing the required knowledge, skills, and abilities to perform the essential functions of the job will be considered.ADA Specifications:Normal office environmentThe work pace is rapid, and concentration required is intermittent with an average amount of detail often causing pressure or the feeling of pressure.
