About This Position

GiaCare Inc. is accepting resumes for experienced, civilian Case Manger (Bachelor's Degree) to work at the Womack Army Medical Center in support of Fort Bragg (near Fayetteville, North Carolina).


This position is responsible for managing the care of active-duty service members on Pope Air Field, Fort Bragg (formerly Fort Liberty). The Case Manager will be assigned a caseload for active-duty service members struggling with mental health and/or substance abuse issues. This will be accomplished by assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs.

QUALIFICATIONS:

Must possess a combination of mental health work experience or relevant education, and training which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties.

  • Bachelor degree or relevant experience in human services or psychology field
  • Life Support Certifications: Basic Life Support (BLS) from an American Heart Association accredited program.
  • Experience: Three (3) years’ work experience within mental health setting. Experience with military mental health is preferred.
  • Computer Skill Competency: Each Case Manager shall demonstrate competency as required.
  • Experience working with an electronic health records system and will be expected to possess computer skills sufficient to satisfactorily perform all tasks required.

Preferred experience but not required:

- Working with Active-Duty Service Members

- Prior active-duty service behavioral health technician

- Certified Alcohol and Drug Abuse Counselor


ESSENTIAL KNOWLEDGE AND ABILITIES:


Knowledge:

Working knowledge of the nature mental illness, substance abuse and related treatment modalities, interventions and techniques; of different types of assessments and their uses in treatment planning; of patients’ rights; of local community resources; of client record documentation requirements; and of client services plan development and implementation.

Abilities:

Demonstrated ability to interview and assess clients, using appropriate assessment tools, and observe, record and report on an individual’s functioning; to read and understand assessments, evaluations, observation, and use in developing treatment plan; to identify community resources and services for clients and coordinate provision of services; to establish effective working relationships with internal agency staff as well as with relevant community organizations; interact positively with patients and their leadership/families, work as a team member, communicate effectively, verbally and in writing, to maintain confidentiality, and to work independently under general supervision.

Job Duties:

  • Serves as Mental Health Case Management Function Leader; assigns and manages staff caseloads.
  • Coordinate appropriate referrals to community and other Government agencies for services.
    • Accesses and manages resources on behalf of service members
    • Advocates for service members through community and government agencies
  • Manages High Interest Program
    • Provides care coordination and follow up planning for individuals being transitioned from higher level psychiatric units and/or follow up from an emergency department visit or medical hospital admission
    • Establishes and maintains high touch communication with high-risk individuals and command teams until a full hand-off and connection is made with the next level of care or provider.
    • Ensures all requirements are met IAW written standards and regulation to ensure safe, timely, quality care
  • Gathers assessment data and facilitates care coordination planning:
    • Reviews all clinical documentation prior to contact with the individual
    • Assists with the development of a collaborative care plan with the individual, care providers, and other team members.
  • Participates in ongoing collection of data for research and process improvement programming.
  • Schedules appointments, answers phones, triages patients as needed.
  • Participates as directed in the screening of new patients within the clinic.
  • Participates in regular interdisciplinary staff meetings held at position location.
  • Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan to help the clients to achieve the stated goals and objectives.
  • Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements.
  • Participates in the orientation and training of other staff.
  • Provides care coordination and follow up planning for individuals being transitioned from higher levels of psychiatric care and/or follow up from an emergency department visit or medical hospital admission.
  • Completes medical record documentation and coding, and designated tracking logs and data reporting as required by local Medical Treat Facility (MTF)/Air Force/Department of Defense instructions, policies, and guidance.
