Make a Real Difference—Four Days a Week

The role of the CCM/RPM Manager is to enhance the quality of patient management, maximize satisfaction, and promote cost-effectiveness. The CCM/RPM Care Manager will focus on ongoing care management and monitoring to help patients with chronic illness, mental health issues, or life-altering conditions, gain access to health programs and community services, as well as decrease Hospital Readmissions and Emergency Department utilization. The CCM/RPM Care Manager will also work in conjunction with all the health services within Healthy Connections, Inc. to develop a comprehensive service delivery system aimed at meeting the needs of patients. Volunteer hours will be required as part of this role, aligning with the organization's mission as a community health network employing community health workers to support underserved populations.

Required Qualifications:

a. LPN, or similar healthcare professional

b. 2+ years of healthcare experience

c. Strong computer skills

d. Experience with electronic health records, preferably CGM/eMDs

e. Some exposure to, and familiarity with, medical terminology.

f. Excellent communication abilities

g. Detail-oriented with strong documentation skills

h. Excellent organizational, written, telephone, and customer service skills are necessary.

Preferred Qualifications:

a. Direct CCM/RPM program experience

b. Care plan development and management

c. Knowledge of Medicare chronic care requirements

d. Experience with remote patient monitoring platforms

e. Medicare compliance and understanding of G05 l 1 requirements

f. Track record of successful patient engagement
