Job Summary:

Under the direction of the Practice Manager, Advanced Practice Provider for the Mobile Clinic, and Facilities, the Mobile Unit Coordinator functions as a Patient Service Representative and Driver supporting mobile clinic operations. This role provides non-clinical support to patients, providers, and care teams while helping improve patient outcomes in a diverse and vulnerable population. Responsibilities include patient registration, scheduling, rooming support, coordination of services, and operation of a 40-foot mobile clinic.
Duties/Responsibilities:
- Perform Patient Service Representative duties including patient registration, scheduling, check-in/check-out, and information gathering.
- Educate patients on insurance enrollment processes and verify coverage details.
- Enter and maintain accurate patient information in the electronic health record in compliance with HIPAA.
- Support care delivery on a mobile clinic with two exam rooms, including patient flow and basic care coordination.
- Drive and operate a 39–40 foot mobile clinic to designated service locations across the Greater Rochester and Southern Tier regions.
- Conduct daily vehicle checks and coordinate maintenance in collaboration with Facilities, including waste management and supply stocking.
- Ensure the mobile clinic is secure, alarmed, and properly set up at the end of each shift.
- Create and distribute monthly mobile unit schedules in collaboration with leadership.
- Coordinate and organize community outreach events, including vaccine, testing, and primary care initiatives.
- Maintain relationships with community stakeholders to support mobile clinic operations.
- Orient and support new mobile clinic team members.
- Complete accurate and timely documentation per organizational policies.
- Demonstrate teamwork and effective communication across departments and care teams

Required Skills/Abilities:
- Understanding of electronic health records (EHR) systems required
- Demonstrated ability to work effectively in a team environment
- Strong interpersonal and communication skills (written and verbal)
- Proficiency in computer systems and basic office software
- Valid, clean New York State driver’s license required and ability tooperatethe mobile clinic vehicle
Education and Experience:
- Experience working in community-based healthcare settings preferred
- Class B CDL license preferred
- Experience driving large mobile units preferred
Physical Requirements:
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
