Position Summary
Elite Home Health is seeking a motivated, organized, and relationship-driven Documentation Specialist to join our growing team. In this role, you'll serve as a vital connection between our clinical teams and referral partners, helping ensure physician orders and documentation are completed accurately and on time.

Key Responsibilities
· Print, organize, and prepare orders for the sales team to deliver to physician offices and referral sources.
· Collaborate with the sales team to establish and maintain routine schedules for order delivery and follow-up.
· Partner with referral sources and physician offices to obtain outstanding documentation in a timely manner.
· Follow up on missing, incomplete, or inaccurate documentation to ensure orders can be processed without unnecessary delays.
· Research and resolve documentation discrepancies, including identifying and contacting the appropriate physician or office to obtain correct information.
· Escalate urgent or time-sensitive orders to the appropriate internal and external business partners to facilitate prompt resolution.
· Build and maintain positive working relationships with referral partners, physician offices, and internal departments to support efficient communication and collaboration.
· Monitor outstanding orders and provide weekly status updates on progress, barriers, and resolution efforts.
· Respond promptly to inquiries, issues, and requests while providing effective solutions and excellent customer service.
· Maintain accurate records and documentation throughout the order management process.
· May assist with on-site document collection at referral partner locations as needed; occasional local travel may be required.
· Ensure all documentation meets company, payer, and regulatory requirements for accuracy and completeness.
· Comply with all agency policies, procedures, standards of practice, and applicable regulatory requirements.
· Perform other duties as assigned to support operational excellence.
Qualifications
· High school diploma or equivalent required; associate's degree preferred.
· Previous experience in healthcare, medical office administration, referral management, customer service, or order coordination preferred.
· Strong organizational and time management skills with the ability to manage multiple priorities.
· Excellent verbal and written communication skills.
· Ability to build and maintain professional relationships with internal and external stakeholders.
· Strong problem-solving skills and attention to detail.
· Proficiency with Microsoft Office applications and electronic medical record (EMR) or healthcare management systems is preferred.
· Ability to work independently while collaborating effectively with cross-functional teams.
· Valid driver's license and reliable transportation may be required for occasional travel to referral partner locations.
