Job Description
Reports to: Case Management Program ManagerFLSA Classification: NonexemptSchedule: Monday – Friday during core business hours of 8:00am – 4:00pm, May require occasional evenings and weekends depending on business needsPrimary Work Location: Hybrid and Springvale, Maine areaUpdated: September 30th, 2026
Summary:Case Managers are part of a team who provide Adult Disability Services Case Management services to individuals with intellectual & developmental disabilities. The position is 40 hours/week with access to paid time off and health insurance. Case Managers report to the Case Management Program Manager and receive support from the Clinical Supervisor as needed.
Essential Duties and Responsibilities:· Advocate for and assistance with applying for MaineCare services including medical, housing, employment, community-based services, Section 21 and 29 DS Waiver funded services and other services as needed.· Meet regularly with members to ensure progress, health and safety, quality of life, and address any concerns in these areas.· Assist with social security, food stamps, MaineCare and financial management.· Help with monitoring and coordinating medical, dental, vision and psychiatric appointments.· Maintain routine contact with members and support team and collateral contacts for ongoing assessment of member needs and safety.· Coordinate, review, and document person centered plans, care plans and behavior plans.· Maintain accurate case records of assessment, activities, and plans.· Maintain EverGreen reporting requirements.· Discuss and document progress, challenges, and barriers to success with the individual and the treatment team.· Evaluate impact and efficiency of services.· Monitor community-based Medicaid Waiver programs and document progress and outcomes.· Participate in crisis resolution using crisis intervention techniques and skills as well as collaborate with Adult Protective Services and Crisis Prevention/Intervention Services, when necessary, to stabilize and secure emergency needs.
Qualifications:· Bachelor’s degree from an accredited four (4) year institution of higher learning with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing, or a closely related field· LSW or LSX preferred· 3 years’ experience in the field of social services· Possess a valid driver’s license, violation free driving record, use of a personal vehicle required· The successful candidate will demonstrate a solid ability to manage time and tasks independently and successfully. They will also be able to learn and apply knowledge of the complex DHHS Regulations and guidelines, along with the timelines associated with support and service needs.· Positive and effective communication skills (oral and written), excellent organizational skills, conflict resolution, problem solving and decision-making skills· The successful candidate will be subject to routine background checks, including driving, DHHS, & criminal. A valid driver’s license, evidence of current vehicle registration, inspection and insurance are also required.
