Summary of Job Description:
Consejo provides integrated, community-based behavioral health services for clients with complex mental health, substance use, and psychosocial needs. This role provides comprehensive case management through a decentralized care model, coordinating mental health, medical, housing, advocacy, and support services. Services are delivered in homes, schools, community settings, and agency locations to promote stability, recovery, wellness, and continuity of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Client Services & Assessment
• Maintain an assigned caseload in alignment with program expectations and required productivity standards.
• Deliver daily direct services, including case management, individual and group interventions, care coordination, phone contacts, and community based services.
• Conduct behavioral health screenings, intakes, needs assessments, and risk assessments to determine appropriate services and level of care.
• Engage clients using trauma-informed and culturally responsive practices.
Crisis & Care Coordination
• Assess and respond to crises, coordinate with providers and emergency systems, and support stabilization.
• Coordinate mental health, substance use, medical, housing, legal, educational, and community services.
• Support discharge, re-entry, and continuity of care while advocating for client needs.
Community & Family Engagement
• Provide mobile services in homes, schools, and community settings.
• Support youth, justice-involved clients, families, and natural supports.
• Facilitate groups, education, prevention, and recovery activities.
Documentation & Compliance
• Maintain accurate documentation, records, and care plans while meeting HIPAA, 42 CFR Part II, and agency requirements.
• Participate in supervision, training, meetings, committees, transportation, and other assigned duties.
QUALIFICATIONS:
• Bachelor’s degree in social work, behavioral science, human services, criminal justice, or a related field, or equivalent combination of education and relevant experience.
• Agency Affiliated Counselor registration required
• National Identification Number Required
• Demonstrated experience working with clients with mental health, substance use, co‑occurring, or complex psychosocial needs
• Demonstrated experience with evidence-based practices, including group facilitation, individual counseling, assessments, and treatment planning.
• Demonstrated cultural competence and commitment to trauma-informed care, including cross-cultural, social, economic, and physical disability challenges when working with multi-cultural clients.
• Experience working multiple systems serving youth, families, adults, and older adults.
• Experience navigating legal, criminal justice, and social service systems effectively
• Valid Washington State driver’s license, reliable transportation, and current automobile insurance
WORK SCHEDULE
This full-time in-person position operates on a standard 8:00 AM to 5:00 PM schedule, Monday through Friday.
SALARY
$27.00 - $33.10/hr. Based on Experience, Licenses, and Education.
