Purpose:
The overall goal of this position is to supervise in ensuring compliance with agency policy and protocol as well as federal and state regulations. The position will be the interface between referral sources, the client and the agency personnel, prior to admission of client to home health care service. This will ensure client’s needs and expectations are properly managed.
Scope & Objective:
To lead in review of all referrals/intakes received by the Intake Department to ensure the referral complies with state, federal and company guidelines. Provides feedback to Clinical Leadership regarding any variance. Helps in assuring quality in the agency’s delivery of care to clients. The Clinical Intake Coordinator acts as initial main contact for staff, client, client’s representatives and other providers as the primary source for admission information and process. Works in collaboration with CARE team and Revenue Cycle with regards to but not limited to consent, patient’s rights, arbitration, contact, financial liability, coverage, expectations, admission process, schedule, and other information that will require admission to home health care services.
To make sure that the agency follows all regulatory, practice, protocol and scope of profession issues of concerns. To effectively coordinate with agency’s administrative and field staff that client has approved and consented the agency to provide home health services at home. To ensure that we acquire the admission documents upon admission. To assure that admission documents and information are complete and accurate and are compliant with agency, state and federal regulations.
Reports to: Director of Operations
Tasks/Duties & Job Responsibilities:
- Supervise Intake team employees and their assigned job functions.
- Provide a team approach to building Horizon Hospice Care's reputation of quality service, dependability, and ownership of delivering great care to our patients, sources, and staff.
- Lead and manage the Intake Department making sure to meet the demands of the client, community and the agency.
- Monitor Starts of Care (SOC) to ascertain if they were completed within 24 – 48 hours. For any SOCs that not completed timely; communicate appropriately to Operations Director, Clinical Leadership, and Scheduling
- Recertification compliance dates and orders within the allowed 5-day window.
- Maintain the accuracy and timeliness of the referral process.
- Maintain open lines of communication with Intake, Clinical Leadership, Scheduling, Marketing, Revenue Cycle, and the patient: all that contribute to the proper processing of a referral.
- Attend meetings (Monthly Administrative Meetings, Weekly Team Meetings, stand-up meetings, etc.) as requested by the Operations Director and/or Clinical Leadership and prepare data for such meetings as required.
- Prepare daily, weekly, and other reports as necessary and as requested by leadership.
- Support other departments by answering questions and providing expertise to these duties listed.
- Other tasks and special projects that may be assigned from time to time.
- Assist the Director of Operations on research projects as may be assigned.
- Provide feedback and recommendations to the Director of Operations to improve effectiveness and efficiency of processes and delivery of care & customer service.
- Assist in interviewing candidates for administrative positions
- May assist in the training and on-boarding of new and/or transferring employees.
- Complete both introductory and annual reviews for intake team members; may collaborate on other agency employee’s reviews as well.
- Responsible for coaching staff and participating in Corrective Action as necessary.
- Promote agency reputation and staff morale by serving both internal and external relationships with professionalism and excellent customer focus/service
- Collaborate with other teams of the agency to promote efficiency and effectiveness of overall agency operations.
- Uphold Horizon Hospice Care Vision and Mission and Core Values of commitment, advocacy, respect, and excellence.
- Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.
- Completes emergency preparedness training as identified as appropriate for this employee level.
- Participates in exercises and drills for emergency preparedness, as required.
- Other duties and tasks as assigned by the Horizon Hospice Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.
- Education: High School diploma or G.E.D required and Bachelor’s degree preferred.
- Experience: Preferred one year of home care experience and intermediate level of experience with Excel. One year of supervisory experience preferred.
