• Perform comprehensive physical therapy evaluations and establish individualized plans of care.
• Develop and implement treatment plans to improve mobility, strength, balance, and functional independence.
• Monitor patient progress and modify treatment plans to achieve optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety, and disease management.
• Collaborate with physicians, nurses, and the interdisciplinary care team to coordinate patient care.
• Complete accurate and timely clinical documentation.
• Maintain compliance with infection control standards, agency policies, and regulatory requirements.

• Graduate of an accredited Physical Therapy program.
• Current, unrestricted Physical Therapist (PT) license.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• One (1) year of Home Health experience preferred.
• HomeCare HomeBase experience preferred.

• Ability to stand, walk, bend, squat, kneel, climb, and sit for extended periods.
• Ability to safely lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
