Role Overview

The LPN is responsible for supervising day-to-day health services activities. Health Services provide services and support for foster care, residential and ACS involved youth..

Key Responsibilities

  • Provides general medical care and treatment to individual supported per program, on site.
  • Performs physical examinations and preventative health measures within prescribed guidelines and instructions.
  • Provide health evaluations, assessments, and screenings with follow-ups.
  • Review all Pediatric guidelines for care, including immunizations and testing
  • Records physical findings based on the patient’s condition.
  • Participate in clinical team and other health professionals to prepare comprehensive patient care plan utilizing historical and current information
  • Conducts and assists in medical examinations of youth after incidents.
  • Provides health education to youth supported, including reproductive health education
  • Administer Medication
  • Train staff
  • Assist with community provider visits as needed
  • Other related duties as assigned by supervisor.

Required Qualifications

  • Current Licensed Practical Nurse (LPN) license in New York State, in good standing.
  • Minimum of 2 years of nursing experience in a clinical, community-based, or related healthcare setting.
  • Current Basic Life Support (BLS) certification required.
  • Demonstrated ability to perform comprehensive nursing assessments and exercise sound clinical judgment in an autonomous work environment.
  • Strong critical thinking and problem-solving skills, with the ability to evaluate and interpret medical records, laboratory reports, and other clinical documentation.
  • Experience triaging patient calls, assessing urgency, and coordinating appropriate follow-up care.
  • Ability to provide guidance and support staff as needed while maintaining high standards of patient care.

Position Type/Expected Hours of Work

Full-time position-hours are 7am – 3pm. Work is conducted in clinical settings within program facilities and residential programs. This is an in-person position only
