Job Title/Position: Hospice RN Case Manager
Reports To: Director of Nursing
FLSA Status: Exempt
Job Description Summary
The Hospice RN Case Manager is an integral part of the hospice clinical team. Under the direction of the Director of Nursing, the job responsibilities include the following:
- Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
- Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
- Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
- Provide regular visits to re-assess the patient's needs and update the plan of care
- Perform clinical pronouncement of death and provide support to the family
- Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
- Participate in all interdisciplinary team meetings to discuss current patient status and ongoing patient care needs
- Initiate medical orders to contracted vendors based on patient care needs (ie; pharmacy, durable medical equipment, medical supplies, etc)
- Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
- Demonstrate flexibility while performing job duties to ensure the highest quality of service is provided
- Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
- Utilize critical thinking skills while performing job duties
- Maintain a thorough understanding and consistently adhere to all applicable department and company policies and procedures
- Conduct job duties in a way which will reduce and avoid potential safety hazards
- Demonstrate initiative and drive to promote personal and company success
- Perform other duties as assigned by your supervisor or management team member
Minimum Eligibility Requirements
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice and in good standing
- Two (2) years experience as a registered nurse in a clinical care setting. Hospice experience preferred.
- Must possess current CPR, First Aid Certification
- Must be knowledgeable of quality assessment and assurance procedures
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
- Valid driver's license and proof of insurance is required
- Background check and drug clearance

Physical/Sensory Requirements (with or without the aid of mechanical devices)
Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients.
Requires working under some stressful conditions to meet deadlines and patient needs, and to
make quick decisions and resource acquisition; meet patient and family individualized
psychosocial needs. Requires eye-hand coordination and manual dexterity.
Environmental and Working Conditions:
Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and
infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to
unpleasant weather; PRN emergency calls.
