Responsibilities
This position is full-time, on-site, and located at our Fort Worth,Texas site. You must be located in Forth Worth, Texas or any surrounding area.
Care Coordinator Specialist
The Care Coordinator Specialist operates under the guidance of a Registered Nurse (RN) to deliver coordinated care services for injured workers managing low to moderate complexity claims. This role focuses on ensuring seamless delivery of medical services, maintaining clear communication among stakeholders, and supporting timely return-to-work outcomes in accordance with workers’ compensation guidelines.
Key Responsibilities
Injured Worker Support
Responsible for completing initial injury reports with injured workers; reviewing the Workers Compensation Program Information packet with injured employees; obtaining signatures for all workers compensation consent forms; authorizing and scheduling initial medical appointments; and addressing general customer service inquiries concerning the workers compensation process for injured employees.
- Conduct face-to-face interviews with injured workers to complete the initial injury report in an accurate and timely manner
- Guide injured workers through the process of selecting an appropriate physician from the Texas Medical Provider Network (MPN)
- Schedule initial medical appointments for injured workers, ensuring prompt access to care
- Obtain required signatures on all necessary workers' compensation consent forms, ensuring full compliance with applicable regulations
Administrative Support
- Monitor and manage all incoming faxes and emails for the workers' compensation office
- Prepare and send medical request forms to physician offices as needed
- Maintain accurate and organized records in accordance with workers' compensation guidelines

Qualifications
Education & Licensure:
- High School Diploma required
- Graduate of an accredited practical/vocational nursing program
- Current compact LPN/LVN license in good standing
Experience:
- 2+ years of clinical experience required
- Workers' compensation or disability management experience is highly preferred
- Healthcare coordination and documentation experience preferred
Skills:
- Demonstrated organizational, technological, and communication skills
- Basic understanding of disability management principles
- Computer proficiency, including Microsoft Office Suite (Word, Excel), keyboard skills, and data entry capabilities
This position is onsite only. Must have reliable transportation
