• Provide skilled nursing care, comprehensive assessments, and ongoing treatment where residents live.
• Develop, document, and update individualized plans of care, including admission and recertification assessments.
• Teach patients and families about medications, self-care, nutrition, and managing health conditions.
• Coordinate with physicians, community staff, families, and our care team when a patient’s needs change.
• Provide clinical guidance and support to LPNs/LVNs and Home Health Aides.
• Participate in community wellness events, health fairs, and resident education sessions.
• Maintain timely clinical documentation and follow infection prevention practices.

• Current RN license
• CPR certification
• Valid driver’s license and auto insurance
• At least two years of home health RN experience preferred
• HomeCare HomeBase experience preferred
