Job Description

  • Responsible for conducting clients’ intake screening and assessments, determining eligibility and treatment needs, developing individualized person-centered service plans, providing counseling and case management services for individuals with substance use disorders and facilitating morning meetings.
  • Ensures that services are delivered in accordance with program standards, regulatory requirements, and best practices for person center client care.
  • Conduct comprehensive intake interviews and substance use assessments using approved screening tools.
  • Gather and verify client information including medical including blood work and PPD, psych-evaluation if needed, psychosocial, and substance use histories.
  • Determine appropriate level of care and eligibility for program services based on assessment findings and verify insurance through E-paces.
  • Collaborate with multidisciplinary teams to develop individualized treatment plans.
  • Maintain timely, accurate, and confidential documentation in the Electronic Health Record (EHR) system.
  • Educate clients on program policies, confidentiality rights, and treatment expectations during intake.
  • Monitor client progress and update treatment plans as needed.
  • Assist clients with referrals to ancillary services such as mental health care, housing, medical, and employment resources.
  • Participate in case conferences, team meetings, and training as required.
  • Ensure compliance with all federal, state, and agency regulations (including HIPAA, 42 CFR Part 2, and relevant licensing standards).
  • Always maintain professional and ethical standards
  • Perform other duties as assigned.

Education and/or Work Experience Requirements:

  • Must have bachelor’s degree of current License, (CASAC-T/CASAC/Provisional CASAC, LMSW/LMHC/MHC-LP).
  • 1- 2 years of experience with addiction client-center treatment and experience in behavioral health.
  • Familiarity with Motivational Interviewing, Behavioral therapies, person-centered treatment, and evidence-based practices for the treatment of addictions.
  • Proficiency and experience with Microsoft Office and Electronic Health Records system.
  • Knowledge of Substance Abuse
Good verbal and written communication.
