North Hills Community Outreach (NHCO) is a nonprofit organization dedicated to supporting individuals and families in crisis by addressing basic needs, promoting long-term stability, and supporting our older adult neighbors. We envision Northern Allegheny County as a sharing community where no one needs to be cold, hungry, or alone.

The North Boroughs Services Coordinator is responsible for providing coordination of services for individuals and families in the North Boroughs service area utilizing a whole family approach. This role supports individuals and families in crisis, helping them stabilize immediate needs and work toward long-term economic self-sufficiency. The service coordinator serves as a liaison between clients, community partners, and internal staff to ensure high-quality service delivery alongside ensuring accurate documentation and reporting in program databases.

Essential Duties

  • Screen callers and walk-in clients for eligibility and appropriateness for Pathways and/or Transportation Financial Assistance programs.
  • Conduct client assessments to identify strengths, needs, and goals, developing individualized service plans to address immediate crises and support long-term self-sufficiency.
  • Provide ongoing case management, including regular follow-up with clients to monitor progress and outcomes.
  • Assist enrolled households with applications for programs such as LIHEAP, Dollar Energy, and Department of Public Assistance.
  • Refer clients to appropriate community resources and organizations as needed.
  • Advocate on behalf of clients to ensure access to eligible services.
  • Maintain accurate and detailed case notes, goal plans, and data entry in Synergy (CSBG-specific, web-based applications) and NHCO databases.
  • Organize community outreach activities and presentations, building relationships with community leaders and partner organizations to facilitate client engagement.

Secondary Duties

  • Collaborate with NHCO staff and external partners to enhance program delivery and community outreach.
  • Participate in team meetings, training sessions, and organizational initiatives as required.
  • Perform other duties assigned to support NHCO programs and client services.
Requirements:

Knowledge, Skills and Abilities

  • Strong interpersonal and communication skills, both verbal and written, with the ability to interact effectively with diverse clients and community partners.
  • Ability to manage multiple tasks and deadlines independently with professionalism and attention to detail.
  • Proficiency in Microsoft Office applications and accurate data entry.
  • Ability to work independently in a community-based setting while maintaining accountability to NHCO and partner agencies.
  • Commitment to NHCO’s mission and values, demonstrating empathy, integrity, and collaboration.
  • Basic office and computer skills, including operating standard office equipment (keyboard, printer, copier, etc.).

Experience/ Education Requirements

  • Bachelor’s degree or equivalent combination of education, training, and relevant experience.
  • Minimum of two (2) years of relevant case management or human services experience required.
  • Experience assisting clients in crisis, completing assessments, and developing service plans preferred.
  • Required to submit Child Abuse Clearance, Criminal Background Check, FBI Fingerprint Clearance, and Mandatory Reporter Training.

Working Conditions/ Physical Demands:

While performing the duties of this job, the employee must be able to:

  1. Work performed primarily in an office setting with occasional community outreach or off-site travel.
  2. Standard office environment with occasional evening or weekend work as needed to meet client needs.
  3. Requires sitting, standing, walking, and occasional bending or reaching and ability to lift up to 40 lbs.
  4. Normal range of hearing and eyesight to record, prepare, and communicate information.
  5. Eye-hand coordination to operate office equipment and computers.
  6. Ability to travel locally for community outreach and client visits as needed.
