Job Description

Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.

SCHEDULE:

Flexible schedule - 3 days a week x 7 hours a day.

SUMMARY:

Family Service League is seeking a part-time Adult Homes Coordinator for our Ombudsman Program. The Adult Homes Coordinator will be responsible for visiting Adult Homes weekly,visiting uncovered nursing homes, assisted living, and FTHA. They will also be responsible for mentoring volunteers.

RESPONSIBILITIES:

  • The Adult Homes Coordinator will be responsible for visiting assigned facilities as well as those that do not have an Ombudsman assigned to ensure residents have access to an Ombudsman.
  • Accurately complete all reports and documentation required by the program and submit by required deadlines.
  • Travel to various long-term care facility locations throughout Suffolk County to provide coverage.
  • The Adult Homes Coordinator will assist staff members and residents to develop resident councils and resident rights training in all Adult Care Facilities.
  • Attend staff meetings, in-service trainings, and LTCOP trainings..
  • Assure compliance with Family Service League policies, standards and regulatory requirements, including the Office of The State Long Term Care Ombudsman.
  • The Adult Homes Coordinator will provide resources and referral information to residents and families.
  • Demonstrate cultural competency with communications and complaint resolution.
  • Obtain NYS Ombudsman Certification.
  • All other duties as assigned.

QUALIFICATIONS:

Bachelor's Degree in human services, social work or related field required.

NYS Ombudsman license/certification required. LTCOP Ombudsman Certification- training provided.

Minimum of 3 years of experience working with older adults and disabled individuals required.

Understanding of aging, health care and long-term care issues preferred.

Computer proficiency, including Microsoft Office, required.

Strong interpersonal skills and the ability to deal effectively with a diverse range of residents required.

Excellent organizational skills, and the ability to manage multiple tasks and meet deadlines required.

Excellent verbal and written communication skills, as well as presentation skills, required.

Valid and clean NYS Driver’s License required.

PHYSICAL REQUIREMENTS:

Some light lifting required.
