Job Description
Case Manager
The City of Champaign Township works to provide essential services with compassion to the community through programs in a fair, equitable and responsive manner. We are seeking a dependable, efficient & proactive employee to assume the position of Case Manager.
Position Summary
Case Manager will be the first point of contact for community members who utilize services at City of Champaign Strides. Services provided by case managers will happen primarily within the shelter facility except events or assigned RRH after move in.
The Case Manager reports directly to the Program Director.
Primary Responsibilities
Case Management
Manage a caseload between 12-15 guests
Meet with guests on caseload weekly or biweekly
Approach guests where they are and build goals and programming alongside
Schedule intakes for guests as requested
Ensure client is not a risk to themselves or others during their time at the shelter and work with staff to determine what next steps should be if they are considered a danger
Assist any guest with immediate case management needs as available
Complete on-going 8-week Internal Programming
Support and attend events, tabling, and external programming
Intakes
Assess community members for need of intakes
Complete full intakes as needed for Centralized Intake and Strides paperwork
Enter case notes with intake meeting notable information
Assist any guest with immediate case management needs of resources or referrals
Complete accurate data collection utilizing WellSky
Data Management
Utilize WellSky to enter all documents, data, and case management/intake hours
Enter case notes with case management meeting notable information
Documentation of case management hours, services, referrals
Complete Strides’ intakes fully
Complete Centralized Intake for Homeless (CIH) referrals when appropriate
Complete and update Individualized Service Plan for guests on caseload
Complete State-wide Referral Network (SRN) for eligible guests on caseload
Update guest information as needed
File all relevant paperwork by the end of your shift & shred unnecessary documents
Complete call log and communication log for your shift
Customer Service
Interact with community members daily
Direct all inquiries appropriately to relevant resources
Answer phone calls, document calls, and notify appropriate staff if necessary
Support guests, visitors, and other staff as needed
Remind guests of the policies in place when they utilize the shelter and hold them accountable
Clean and sanitize facility daily
Resources and Research
Locate additional resources relevant to shelter guests
Create or research systems for case managers to utilize
Search for Professional Development opportunities for self, Case Managers, or Strides’ staff
Complete assigned Professional Development
Other Required Duties & Responsibilities
Utilize Microsoft Outlook to notify other employees when in/out/unavailable
Work with Safety to discharge guests if they are unsafe or unable to follow the shelter’s policies
Enforce breaks, service pauses, sanctions, and trespass
Attendance at Required Meetings
Weekly Case Manager Meeting
Monthly All-Staff Meetings for Strides
Monthly one-on-one meeting with Supervisor
Minimum Qualifications
Three (3) or more years of experience in social service entities OR a combination of education and experience
At least 1 year of social service experience
Active certifications or willing to be trained for:
CPR & First Aid Certified
Mental Health First Aid Certified
Narcan administration
Trauma Informed Care
Special Skills & Knowledge
Demonstrated ability to quickly build relationships and engage with others
Proficient in Microsoft Office programs
Strong technical aptitude
Excellent communication skills
Excellent customer service skills
Always maintain professionalism
Crisis response or de-escalation trainings or educations
Experience working with underrepresented populations
Able to accept and act on constructive feedback regarding job duties
Able to separate personal emotions from professional duties
Ability to multi-task, prioritize and manage time effectively
Adhere to the confidentiality policy
Strong organizational skills
Be comfortable interacting with various groups of populations that can include, but is not limited to, EMS, police, other organizations, and those guests we serve directly
