Position Type / Schedule
Full-time, 35 hours per week; in-office position with potential field work as needed.
Position Summary
The Care Manager Assistant supports the CT Home Care Program unit through administrative, client-facing, and program support responsibilities. The position may interact directly with clients in the office, in the field, or electronically under the direction of Care Managers.
Essential Duties and Responsibilities
- Creates, updates, and produces client charts.
- Responds to phone and/or email inquiries concerning the CHCP within confidentiality guidelines.
- Greets visitors who come into the office, including clients, family members, and providers, and provides on-site assistance and guidance.
- Scans provider reports and other relevant case documentation that need to be digitized.
- Processes incoming USPS mail for urgency; sorts, scans, and files documents in the client record.
- May participate in developing reports, monitoring services, and setting up meetings.
- Works with Care Managers to ensure files are complete, including all communication documents relevant to client care and environment.
- May work directly with clients, under the supervision of Care Managers, to complete eligibility documents in the office, in the field, or electronically as needed.
- May provide support in crisis management; assist with mandated reporting, direct care service referrals, and monthly monitoring contacts as directed by Care Managers or supervisors.
- Acts as an administrative liaison with field offices and personnel.
Required Qualifications
- At least two years of demonstrated experience in a work environment that includes service delivery
- Proficiency in Microsoft Office Suite
- Familiarity with computers required, as well as capacity to carry a laptop
- Candidates must be willing to work full-time on-site from the office, with the potential need to work in the field. Travel is required in CT. A personal vehicle for reliable transportation, a valid driver’s license, and current automobile insurance are required.
Preferred Qualifications
- BA/BS degree preferred
- Bilingual English/Spanish language ability is strongly preferred
Compensation: Starting at $35,000 annually
