CHMC –MHU- Treatment Coordinator – Per Diem- Day Shift – 8a-4:30p-Requisition 7750

Pay Range: $26.40-$34.31*

A Mental Health Treatment Coordinator plans and evaluates patients’ care, prepares treatment plans, updates and collaborates with a multidisciplinary team, applies psychotherapeutic techniques in a group setting, conducts family and patient psycho education, facilitates family participation, contacts outpatient providers to facilitate the continuity of care in the inpatient setting as well outpatient follow-up upon discharge.

Qualifications:

• Knowledge, understanding, and ability to work with mentally ill patients required.

• Basic computer knowledge, maintaining confidentiality of all patient information and maintaining quality assurance and compliance with all regulatory agencies required.

• Ability to implement and support procedures and program pertaining to patient safety and security.

• Exhibit proficient communication skills and ability to deal efficiently and effectively with a variety of personalities.

• Ability to sustain attention to detail and accuracy as it pertains to the monitoring and documentation of information. Maintaining a safe and clean environment for patients.

• Bachelors degree in Psychology required. Masters degree in Social Work preferred.

• BLS certification and PMCS training required.

• Valid New York State Master Social Worker license, if position is held by a LMSW.
