About This Position
Statement of Purpose:
Administers nursing care to designated patients under the direction and supervision of a registered professional nurse and/or physician/physician extender. Performs a variety of direct and indirect patient care duties as qualified by licensure, orientation, and competency.
MAJOR TASKS, DUTIES AND RESPONSIBILITIES
- Maintains the standards of nursing care in the practice and implements the policies and procedures.
- Monitors, documents and reports all observed signs and symptoms, responses to treatments, changes in patients condition, pertinent observations and reactions to the physician/physician extender.
- Administers medications at the direction of the physician/physician extender; transmits prescriptions to pharmacies via telephone, fax or e-prescribe and documents the information as required in the patient’s medical record.
- Assists provider with special tests and procedures as mandated by the specialty of the practice.
- Collects specimens for lab analysis and performs waived testing per JMH protocol, collection to include venipuncture for obtaining blood sample upon completion of outpatient lab orientation.
- Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner. Orders and stocks supplies as needed.
- Participates in orientation of new personnel. Assists team members in giving care to patients, or administers direct care when professional nursing skills and judgment are needed.
- Establishes and maintains communications and utilizes teaching opportunities with patients and other significant individuals.
- Obtains prior authorizations, makes referral appointments for specialists or follow-up testing per provider orders.
- Assists with emergency measures for sudden adverse developments in patients such as cardiac arrest. Maintains current CPR/BLS Certification.
- Performs duties and assists providers as part of the practice-care team. Takes part in daily team huddles and/or other periodic team meetings to facilitate communications and enhance position patient experiences and quality delivery of care.
- Participates in quality improvement initiatives as part of the practice care team.
- Ensures job functions align with office policies, procedures, and patient-centered medical home standards.
- Demonstrates knowledge and skills necessary to provide care appropriate to all ages of patients as documented by training and competency assessment.

When staffing or patient care needs dictate, may work hours outside of normal schedule and/or in another practice.
EDUCATION: Graduate of an accredited school of practical nursing. New York State licensure and current registration as LPN required. Current CPR/BLS certification. If working in URMC specialty clinic, will need additional training in EPIC.
EXPERIENCE: One-year acute care and/or one-year doctor office experience.
CONTACTS: Frequent contact with patients, family members, physicians, and other employees.
PHYSICAL DEMANDS: Stands and walks most of the day.
WORKING CONDITIONS: Normal. Frequent exposure to disagreeable odors, cuts, burns, infections, communicable diseases, strains and injuries from patients and equipment.
