Position Summary
The Licensed Practical Nurse may perform selected acts in accordance with the Nurse Practice Act, including the administration of treatments and medications in the care of the ill, injured, or infirm, the maintenance of health, and prevention of illness, under the direction of a Registered Nurse. The LPN provides service in accordance with the Agency philosophy, standards and policies.

Essential Functions
- Provides services in accordance with Agency policies.
- Assists the RN to perform specialized procedures and carry out the plan of care.
- Prepares clinical and progress notes within the specified timeframe of the Agency.
- Assists the RN in performing specialized procedures and duties including: preparing equipment and materials for treatments, observing aseptic technique, and assisting the patient and family member in learning the appropriate self-care techniques.
- Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Functions as a team member demonstrating collaboration with, and responsibility to patient, physician, interdisciplinary team
- Performs patient care responsibilities considering needs to the standard of care for patient’s age.
- Promotes patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic and/or personal needs or preferences.
- Attends 80% of mandatory in-services and case conferences to discuss multidisciplinary team responsibilities and patient progress.
- Collaborates with the Case Manager on upcoming Supervisory Visits (every 30 days).
Professional Requirements
- Maintains patient confidentiality and HIPAA standards at all times.
- Adheres to legal practice standards, including applicable Federal, State and local regulations and standards.
- Completes annual education requirements.
- Demonstrates effective time management and organizational skills; provides patient visits as scheduled, and completes patient care within scheduled times.
- Attends annual review and in-services as scheduled.
- Maintains currency of professional knowledge by participating in continuing education offerings.
- Assures continued competency by maintaining and updating professional performance, knowledge and skills.
- Practices in a manner consistent with professionalism and Agency Code of Ethics.
- Represents the organization in a positive and professional manner.
- Complies with all organizational policies regarding ethical business practice.
- Communicates the mission, ethics and goals of the organization.
Regulatory Requirements
- Graduate of an accredited nursing program. Current State licensure as an LPN.
- One year of experience in a clinical care setting, home health preferred, within the last 24 months.
- Six months of home care experience within the last 12 months.
- Current CPR certification, proof of Tuberculosis PPD given within the past 12 months, and Hepatitis consent/declination.
- Valid state driver’s license, reliable automobile and proof of current automobile insurance.
