Independent Living Specialist ($1000 Hiring Bonus!) at Placement Partners MN, Inc – Edina, Minnesota

Job Summary

Assist consumers to develop, maintain and improve the community living skills. Services shall be delivered in a manner that promotes individual success, independence and respect. The Skills Trainer shall cooperate as a member of the Interdisciplinary Teams and work in liaison with the Residential Associate Program Director, when necessary, to resolve issues that may arise in the training process.

The Independent Living Specialist shall assist the consumer in developing, maintaining and improving his/her day to day activities necessary
to live independently and successfully within the community.

1. Provides weekly one on one support assisting the consumer with activities of daily living;

2. Helps the consumer with developing healthy lifestyle habits and engaging within their communities;

3. Demonstrates creative problem solving skills to help his/her consumer attain their personal goals and meet life’s challenges;

4. Assists the consumer with accessing and navigating through community resources.

Companion Responsibilities:

The Independent Living Specialist shall provide support to his/her consumer and demonstrate examples of interpersonal behaviors out in the
community.

1. Accompanies the consumer on outings within the community;

2. Serves as an advocate for the consumer with his/her service providers;

3. Provides ongoing training and encouragement to the consumer with making healthy and responsible life choices;

4. Other individual services, as necessary, to assist the consumer in living independently in the community.

Job Qualifications

Must be at least 18 years old, have a high school diploma/GED, have a valid driver's license with a reliable vehicle and proof of up to date auto insurance. Prior experience working with individuals that have developmental disabilities in a social services field is not required but preferred.

Expected Hours: 40 hrs (Full Time)
