GENERAL POSITION DESCRIPTION:
Provide case management services to clients approved for the Colorado Works Program and Child Care Assistance Program (CCCAP) benefits according to State and County regulations. Communicate the purpose and expectations of the Colorado Works Program, aka Temporary Assistance for Needy Families (TANF), to individuals approved for assistance. Conduct assessments on Colorado Works clients to identify job readiness, strengths and barriers and develop an Individualized Plan (IP) to work toward self-sufficiency. Determine initial and continuing eligibility for CCCAP. Determine eligibility for the County’s General Assistance Program.
- Level I – Applies basic skills while developing some specialized skills in procedures, operations, techniques, tools, materials, and/or equipment appropriate to area of specialization; performs routine and/or repetitive tasks, limited decision making required; generally, works under close supervision; has no or limited experience; work is performed from within specific limits of established procedures and/or defined instructions. (generally 0-3 years of experience)
- Level II – Applies skills and job knowledge in area of specialization; applies procedures, operations, techniques, tools, materials, and/or equipment to meet needs of area of specialization at an intermediate level of knowledge and understanding; may work on non-routine tasks, projects, and applications; resolves issues/makes working decisions within area of specialization or responsibility with minimal supervision. (generally, a minimum of 2 years of experience in the role)
- Level III – Works on varied tasks ranging from routine to complex. Applies full range of specialized skills and job knowledge; frequently adapts procedures, techniques, tools, materials, and/or equipment to meet specialized needs; may serve as lead; performs broad and/or focused assignments under general supervision; relies on experience and judgment to plan and accomplish assigned tasks. (generally, a minimum of 4 years of experience in the role in Delta County)
ESSENTIAL FUNCTIONS/ RESPONSIBILITIES:
- Facilitate individual and family assessments that help identify job readiness, individual strengths, and barriers to self-sufficiency; regularly assess client needs/barriers and revise the Individual Plan (IP) to achieve short and long-term goals specific to becoming self-sufficient. Explore options and resources available to assist the client meet basic family needs.
- Ensures all Colorado Works recipients are in a State or County defined work activity while receiving cash benefits; monitor work activity attendance/participation and progress; timely and accurately document clients activities in the Colorado Benefit Management System (CBMS) to ensure proper reporting as required by program rule.
- Assist Colorado Works clients in developing an Individualized Plan (IP) that details their short and long-term goals, discuss and evaluate any possible employment barriers, communicate program expectations, and assist with identifying community resources offering supportive services; utilize case management techniques to ensure IP engagement, re-assessing, and re-evaluating client needs in order to move the client toward a self-sufficiency status within a reasonable amount of time; take appropriate action when the client willfully is non-compliant or refuses to participate with program requirements.
- Develop and maintain contacts with community organizations and educational institutions for the purpose of outreach, recruitment and positive public relations; make appropriate referrals to outside agencies to assist individuals in meeting workforce requirements as well as for additional supportive services.
- Assist clients in identifying and overcoming barriers that may be keeping them from successful self-sufficiency outcomes such as employment, education, and/or applying for Social Security; document supportive services the client receives from all community resources and Human Services.
- Determine initial and ongoing eligibility per program rules for CCCAP applicants/clients; effectively communicate program functionality to assist the client and provider streamline the monthly process to ensure providers are paid accurately and timely each month.
- Obtain and maintain current Colorado Works and CCCAP knowledge of County/State program policies, procedures and guidelines; adhere to established time frames as set in State and Federal rules and regulations.
- Monitor child care usage to ensure providers are paid accurately via ATS (Automated Tracking System) and parent fees; communicate with child care providers when care is not being utilized as authorized.
- Determine eligibility for the County’s General Assistance Program according to County policy and the requested need; review requests and recommend disposition with the Self Sufficiency Administrator; communicate with providers to verify need; contact community resources on behalf of the client and refer to other resources, as needed.
- Process General Assistance payment approvals in a timely fashion to ensure the emergency is addressed and the provider is paid expeditiously; provide a denial notice to the applicant if program criteria is not met.
- Report suspected fraudulent activity according to County processes.
- Perform case and/or provider reviews as required by the CCCAP Audit division.
- The primary location of employment for this position is the designated Delta County office with the exception of required duties that are to be performed off site as necessary. The employee is required to perform the majority (generally, at least 95%) of assigned duties from this designated office location to allow for client, customer and coworker access and collaboration, unless prior written approval for an alternative arrangement, such as temporary remote work, has been granted by the employee's direct supervisor and/or department head and maintained in the employee personnel file.
- Reliable and predictable attendance and punctuality are considered essential functions of this job. Employees are expected to be at their work area and ready to work at the scheduled start time to ensure consistent service delivery and operational efficiency.
GENERAL JOB DUTIES:
Level I
- Learn the functions of the Front Desk Administrative Assistant position to provide coverage as needed.
- Learn the functions of the Eligibility Technician position to process applications, redeterminations (RRRs), and changes for Medical Assistance, SNAP, and Colorado Works programs.
- Successfully complete the Colorado Child Care Assistance Program (CCCAP) training to begin processing applications, redeterminations (RRRs), and changes in the CHATS system.
- Communicate effectively verbally and in writing; follow and communicate verbal and written instructions.
- Establish effective working relationships with customers, coworkers and supervisors; be sensitive to individual values and needs.
- Become familiar with program rules and regulations for all assistance programs worked to ensure accurate eligibility determination; exercise good judgment in applying and selecting appropriate program rules and regulations for timeliness and accuracy; comply with all program requirements per State and federal regulations.
- Learn the General Assistance Program rules and requirements in order to effectively meet with clients applying for emergency assistance; communicate client needs and recommend a disposition for eligibility to the Self Sufficiency Administrator.
- Treat everyone with professionalism and genuine respect.
- De-escalate upset/angry customers; professionally navigate difficult conversations.
- Should the Board of County Commissioners or another authorized representative issue a local disaster declaration in Delta County, all Delta County employees may be called upon to serve as disaster service workers.
- Perform similar and related duties as assigned.
Level II
- Complete all duties of level I at an intermediate level with minimal supervision.
- Continue to participate in State training, conferences, and meetings to develop more in-depth skills and increase knowledge of the Colorado Works Program, CCCAP, SNAP, and Medical Assistance.
- Demonstrate enhanced case management skills and techniques by assisting the client in identifying short and long-term goals that are specific and attainable; revisit these goals regularly and modify as necessary to achieve success.
- Collaborate with management in program planning and strategies for meeting Colorado Works program goals.
- Participate in State work groups as they become available to provide input from a medium county’s perspective and contribute to program enhancements.
Level III
- Complete all duties of level I and II at an advanced level with only general supervision.
- Provide guidance and mentor new staff in learning and working Colorado Works, CCCAP, SNAP, and Medical Assistance programs.
- Perform duties at an advanced level, similar to a lead case manager, by assisting the Self Sufficiency Administrator to monitor Colorado Works and CCCAP reports and ensure timely and accurate processing.
Education:
- A high school diploma or equivalent is required.
- Bachelor’s degree from an accredited college or university in human services, behavioral science, social work psychology, sociology, career counseling, or a related field is preferred.
Experience/Abilities:
Level I
- Experience determining eligibility for Human Services programs is preferred.
- Experience working with low-income, unemployed, under-employed and/or diverse populations is preferred.
- Certificate of completion to process Medical Assistance, SNAP, Colorado Works and is strongly preferred.
- Bilingual in Spanish and English languages is preferred.
Level II
- Demonstrated ability to perform all aspects of Case Manager I expectations at an intermediate level.
- Case Management training certification from the State of Colorado for DEWS Case Management Play List.
- Has attended at least four (4) independent training programs from Colorado Works forums or similar case management concept training AND has demonstrated application of the concepts learned in client case management plans.
- At least one (1) year social case management is required.
- Ability to process SNAP, Colorado Works, Medical Assistance, and CCCAP with 95% accuracy and timeliness is required.
Level III
- Demonstrated ability to perform all aspects of Case Manager II duties independently with little to no oversight.
- Process all programs consistently above 95% accuracy and timeliness at an advanced level.
- Has assisted in training employees in Colorado Works, CCCAP, SNAP and Medical Assistance program eligibility.
- Has monitored Colorado Works and CCCAP reports to ensure timely and accurate eligibility processing and case management plans for the unit.
- Initiated at least two (2) unit or process improvements in the past year.
Combination of education and experience could waive a specific requirement at initial offer of hire.
