Job Summary
The Physical Therapist administers physical therapy services to patients according to the American Physical Therapy Association and Wyoming State Physical Therapy Standards of Practice. The PT is responsible for evaluating assigned patients and establishing, implementing and overseeing appropriate plans of care for these individuals in order to restore and maximize their function and quality of life.

Essential Duties

Practice in a safe manner that minimizes risk to patients, self and others.

Perform patient evaluations and develop appropriate plans of care according to applicable laws and regulatory requirements.

Establish with the patient appropriate and realistic short and long term functional goals.

Administer effective and appropriate treatment interventions that are individualized and adjusted to the needs of each patient for the purpose of meeting their functional goals.

Select and use appropriate, accepted, standardized tests and outcome measures to assess the results of interventions administered.

Communicate effectively and on a regular basis with patients, caregivers and fellow healthcare team members to promote optimal healthcare delivery and progress toward the patient’s functional goals.

Complete documentation related to physical therapy practice in an appropriate, legible and timely manner that is consistent with all applicable laws and regulatory requirements.

Supervise assistive personnel and students in a manner that assures safe and efficient care.

Attend meetings and serve on committees as assigned.

Attend scheduled area daily huddle (TCU at SGH, IP, OP, HH)

Provide patient care as assigned and according to established standard work maintaining 75% productivity.

Evaluate new patients, establish plan of care and document patient goals according to the designated method and timeline once the order is submitted.

Participate in the goal presentation to review goals with new patient.

Perform daily treatment as appropriate to each patient's abilities, needs, and goals.

Represent the best interests of patients as appropriate at patient/family care conferences and IDT meetings.

Complete home evaluations for patients as necessary.

Complete patient care documentation (Evaluation, treatment order, daily notes, progress notes and discharge summaries) in a timely fashion.

Make corrections to documentation within twenty-four hours of notification.

POSITION QUALIFICATION — Education, Experience & Licensure

Master’s or Doctorate in Physical Therapy from a nationally accredited school of physical therapy program.

Maintain current State of Wyoming licensure through the State Board of Physical Therapy or ability to obtain and maintain.

Previous physical therapist experience, preferred

BLS, required.

Central Registry, required.

Additional Skills

Ability to implement physical therapy treatments and rehabilitation activities.

Ability to communicate both orally and in written form.

Ability to assess and evaluate individuals for change in condition.

Ability to plan and organize effectively.

Computer skills.



Specific demands not listed: Possible exposure to blood and or body fluids / infectious disease / hazardous waste requiring the use of Personal Protective Equipment. Exposure to odorous chemicals / specimens and Latex products.
