About the Role

As a Hospice Care Assistant (CNA), you'll play an important role in supporting our mission of providing exceptional care and service. You'll join a collaborative team of professionals who are passionate about making a difference every day.

❤️ What You'll Do

🌿 Provide Compassionate Patient Care
  • Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene.
  • Provide assistance with positioning, transfers, ambulation, and mobility needs.
  • Offer companionship and emotional support to patients and families.
  • Respect each patient's dignity, privacy, preferences, and individual routines.
  • Encourage independence and provide care in a way that promotes comfort and confidence.
❤️ Promote Comfort & Quality of Life
  • Provide comfort measures according to the patient's individualized plan of care.
  • Observe patients for changes in condition, behavior, pain, or comfort level and promptly communicate concerns to the supervising nurse.
  • Assist with creating a peaceful, safe, and comfortable environment.
  • Provide compassionate presence and reassurance to patients and loved ones throughout the end-of-life journey.
🤝 Communicate & Collaborate
  • Work closely with the RN case manager and interdisciplinary hospice team to support individualized patient care.
  • Communicate changes in patient condition or needs to the appropriate member of the clinical team.
  • Build trusting relationships with patients, families, caregivers, and members of the hospice team.
  • Follow the patient's plan of care and perform duties within the scope of your certification and role.
🏡 Support a Safe & Comfortable Environment
  • Assist with maintaining a clean, organized, and safe patient environment.
  • Change linens and assist with basic household tasks related to patient care as appropriate.
  • Follow infection prevention, safety, and patient privacy standards.
  • Complete accurate and timely documentation of care provided.
Qualifications
  • Current CNA certification in the state of Ohio.
  • Minimum of one year of experience working as a CNA in hospice, palliative care, or a similar healthcare setting.
  • Valid driver's license and reliable transportation.
  • Clean driving record and current auto insurance.
  • Ability to travel throughout the assigned service area.
  • Strong communication, teamwork, and interpersonal skills.
  • Compassionate, patient-focused approach to care.
