POSITION SUMMARY:

The Licensed Clinical Social Worker serves as a hospital-based Lead Care Manager and behavioral health subject-matter resource for eligible Inland Empire Health Plan (IEHP) members enrolled in California Advancing and Innovating Medi-Cal Enhanced Care Management. The LCSW provides person-centered, trauma-informed, culturally responsive, and community-based care management across physical health, behavioral health, substance use, oral health, long-term services and supports, pharmacy, and health-related social needs. The role builds trusting relationships with members, coordinates services across systems, supports safe transitions from hospital to community, and practices within the scope of a current California LCSW license.

The Licensed Clinical Social Worker must be able to demonstrate the knowledge and skills necessary to provide information and care to the following age groups: newborn, infant, toddler/preschool, school age, adolescent, adult, and seniors. They must demonstrate knowledge of the principles of growth and development over the life span of the patient and communicate/instruct patients and family members appropriately. They must possess the ability to assess and interpret data reflective of the patient’s status, requirements, and age –specific needs and adjust care and instruction consistent with the developmental level and the unit’s policies and procedures.

REQUIREMENTS:

LCSW with preferred two (2) years’ work experience as an LCSW. Must possess assessment skills, exhibit sound judgment and be able to communicate effectively and Basic Computer Skills. Must be able to write legal holds for psychiatric and gravely disabled holds.

Possess skills to ensure every effort is directed toward helping patients and families develop an adequate continuing care plan in preparation for discharge from the hospital. Must be knowledgeable of community resources and able to network within the community.

Must be able and willing to conduct orientation and in-service classes for staff.

Other Skills:

  • Superb verbal and written communication skills to share updates with staff, patients, and other medical staff.
  • Computer skills and proficiency in Microsoft Office.
  • Strong analytical thinking and the ability to manage multiple tasks concurrently.
  • Excellent customer service skills to provide ongoing support for staff, patients, and their families.
  • Able to manage and deal with stressful situations, work effectively in stressful situations.
  • The ideal candidate will have strong clinical knowledge, excellent organizational skills, and the ability to thrive in fast-paced, high-pressure environments.
  • Demonstrates an attitude of co-operation and professionalism.
  • Consistently demonstrates positive interpersonal relation skills.
  • Demonstrates personal attributes and behaviors which contribute to the smooth operation of the department and/or hospital.
  • Maintains positive physician, administrative, and staff relationships and actively participates in problem-solving. Offers and is open to receiving constructive feedback.
  • Treats everyone, regardless of position, with dignity, respect and courtesy utilizing the principles of Civility. Respects ideas of others, even if they do not agree and offers support.

PAY / BENEFITS
• Estimated pay is based on qualifications (including but not limited to experience, education, certifications, and licensure where applicable).

• Benefits may include medical, dental, vision, basic life and AD&D, optional supplemental life (employee, spouse, and/or child coverage), flexible spending accounts (medical and dependent care), 403b retirement with dollar for dollar match up to 5%, paid time off, and more available depending on employment status.
