Job Title: Hospice CNA – Weight Team Specialist

Location: Service Area (Field / Patient Homes)

Schedule: Monday- Friday, 8:00 am to 5:00 pm

We are seeking a dedicated, physically strong, and reliable Certified Nursing Assistant (CNA) to join our Weight Team. In this specialized role, you will travel to hospice patients' private homes to collect accurate weight measurements using specialized equipment, while also providing essential personal care and hygiene support as outlined in the patient’s plan of care when needed.

• Weight Monitoring: Transport, load, and unload a Hoyer lift scale to and from your personal vehicle and into patient homes to accurately measure and document hospice patient weights.

• Direct Patient Care: Occasionally assist patients with personal hygiene, bathing, hair/skin/mouth care, dressing, feeding, light meal preparation, and toileting needs.

• Mobility & Transfers: Safely turn, reposition, and transfer patients using standard durable medical equipment (Hoyer lifts, wheelchairs, commodes).

• Observation & Reporting: Observe patient physical and mental status; promptly report any changes, deviations, or unusual events to the Staff RN.

• Environment & Safety: Maintain a clean, safe environment for patients.

• Documentation & Compliance: Maintain accurate visit records, adhere to agency policies, and attend mandatory continuing education/in-service training.

• Certification: Active Certified Nursing Assistant (CNA) certification with the State of Alabama.

• Heavy Physical Effort: Must be able to regularly lift, maneuver, load, and unload heavy equipment (including the Hoyer lift scale) in and out of a private vehicle independently.

• Transportation: Must have a reliable personal vehicle, active driver’s license, and auto insurance to travel to patient locations. (Note: CNA will not transport patients in personal vehicles).

• Reliability: Excellent attendance, punctuality, and time-management skills.

Valid CNA license

A valid driver's license

Valid Car Insurance

BLS / CPR Certification: Required for all staff holding a clinical license or credential (Physician, NP, RN, LPN, CNA, or MA), including those in administrative, office, or non-direct care roles. Note: BLS Certification must include an in-person, hands-on skills evaluation (e.g., AHA HeartCode or American Red Cross). 100% online courses without a live skills check will not be accepted.
