Northeastern Vermont Regional Hospital (NVRH) is seeking a Community Health Worker (CHW) to join our team. This rewarding role focuses on improving access to healthcare and social services for community members while helping individuals overcome barriers, navigate complex systems, and achieve their personal health goals. The Community Health Worker serves as a trusted resource and advocate, empowering individuals and families through education, support, and connection to community resources.

Please note: This position is grant-funded and is expected to remain active as long as grant funding is available.

About This Role

As a Community Health Worker, you will work directly with individuals and families throughout the NVRH service area to assess needs, identify resources, and support clients in achieving improved health and well-being. You will collaborate closely with healthcare providers, community organizations, and members of the Community Health Team to ensure coordinated, patient-centered care. This role is ideal for someone passionate about community outreach, health equity, social services, and helping individuals navigate the challenges that affect their health and quality of life.

Key Responsibilities

  • Assess client needs and provide information, support, education, advocacy, and resource navigation services to improve access to healthcare and social services.
  • Empower individuals and families to identify goals, problem-solve barriers, and implement strategies that support self-management and long-term well-being.
  • Collaborate with community organizations and healthcare partners, including community health, social service, public health, and aging service agencies to coordinate services and support clients.
  • Maintain accurate documentation and participate as an active member of the Community Health Team, attending meetings, trainings, and care coordination activities.
  • Support team-based care through the use of motivational interviewing, self-management planning, patient engagement strategies, and care coordination practices.

Required Qualifications

  • High school diploma or equivalent required.
  • Experience working with healthcare agencies, community organizations, or social service programs.
  • Excellent communication and organizational skills.
  • Experience working with women, families, or vulnerable populations preferred.
  • Passion for supporting individuals in achieving improved health and quality of life.

Preferred Skills

  • Experience using computer systems and electronic documentation tools.
  • Knowledge of community resources, healthcare systems, and social service programs.
  • Familiarity with motivational interviewing and patient engagement techniques.
  • Ability to manage multiple priorities and remain calm in stressful situations.
  • Strong interpersonal skills and the ability to build trusting relationships with clients, families, and community partners.
