Job Summary:

Under the supervision of the Program Manager/Clinical Supervisor located at LAC+USC Medical Center HUB Clinic, the Client Care/ Intake Coordinator will provide acute case management services, referrals, and linkage. This role will interact with the community and specific allied agencies and be responsible for managing VIP-CMHC referrals. The Client Care/ Intake Coordinator is responsible for processing and following the referrals until the case has been assigned/scheduled or referred out. This position also liaises between DCFS and other community-based organizations.

ESSENTIAL DUTIES & RESPONSIBILITIES

Collaborative Teamwork

· Work harmoniously within interdisciplinary teams to provide ongoing clinical consultation to DCFS and DMH.

· Establish and nurture relationships with DCFS workers while communicating openly with community-based organizations and collaborators.

Cultural Competence

· Demonstrate a deep understanding of and sensitivity to our client's diverse cultural and socioeconomic backgrounds.

· Incorporate cultural competence in all interactions and interventions.

Youth and Family Support

· Identify strengths and needs of youth and families, considering their unique circumstances.

· Connect clients to appropriate care services and resources within the community, leveraging natural support.

Clinical Documentation

· Maintain accurate and timely documentation, adhering to regulatory guidelines such as DMH requirements.

· Enter progress notes into the electronic record system (Carelogic) within 48 business hours.

Collaborative Care Coordination

· Collaborate with DCFS Clinical Social Workers to ensure seamless coordination of care services for children and youth.

· Facilitate access to essential resources and services by assisting in obtaining past mental health records, court documents, and related paperwork.

Resourceful Crisis Management

· Work effectively in a fast-paced environment, demonstrating strong time management skills and crisis intervention abilities.

Training and Compliance

· Participate in VIP's in-service training programs to enhance professional skills.

· Adhere to agency policies and procedures while performing all assigned duties.

Requirements:

ELIGIBILITY REQUIREMENTS

· Must be willing to submit to a background investigation

* Must be fluent in English and Spanish

· Must have unrestricted work authorization to work in the United States

Knowledge, Skills, & Abilities

· Follow agency policies and procedures.

· Participate in VIP in-service training programs.

· Scan client documents into the electronic health record system

· Provide translation services to clinical staff.

· Demonstrates teamwork and open communication.

· Perform other duties as assigned.

BASIC QUALIFICATIONS:

· Bachelor’s degree and one (1) year documented experience in a mental health setting OR Associate of Arts degree plus two years (2) years documented experience in a relevant mental health setting OR High School Diploma with three (3) years documented experience in a relevant mental health setting.’

· 3 years of documented experience in a mental health setting

· Knowledge in working with the DMH Integrated (IS), IBHIS, and Department of Healthcare Services Medi-cal;

· Spanish Speaking

Supervisory Responsibility

This position will not have any supervisory responsibility.

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

Position Type/Expected Hours of Work

This is a full-time (M-F) in office position and work hours will vary based on department and agency needs but will typically fall within operating hours: 7:30am- 4:00 pm. Schedule is subject to change based on agency needs.

Company Property

A desktop commuter is required to perform the essential functions of this position and will be issued to the employee for business-related use.

Travel

No travel is expected for this position.
