Job Description

The Asian American Center of Frederick (AACF), a minority-led 501(c)(3) community-based organization whose mission is to enrich, equip, and empower immigrants, is seeking a Community Health Worker Coordinator for our expanding Community Connections Hub. AACF provides culturally responsive health and social services to more than 3,000 individuals annually, maintains a multilingual workforce serving 30+ languages, and is a Maryland Department of Health–accredited Community Health Worker Training Center.

As our Community Health Worker Coordinator, you will lead a team of CHWs day to day. This is a working supervisor role: as Coordinator, you will supervise and coach other CHWs while also taking on the Hub's most complex cases and carrying a reduced caseload of your own. Your role will be closest to the daily work of CHWs, and you will work closely with the Director of Community Health, the Consultant Nurse Practitioner, the Behavioral Health Consultant, and AACF's data and IT staff. This role is best suited for an experienced CHW or Social Worker with at least 2 years of experience managing a team.

Key Responsibilities

Supervision and coaching

  • Hold one-on-one check-ins with each CHW at least every two weeks; lead team case reviews and care huddles.

  • Assign and balance caseloads using the bounded caseload framework; monitor timeliness of follow-up.

  • Observe CHW practice in the field and give specific, supportive feedback; onboard and train new CHWs.

  • Review Salesforce documentation for quality and completeness.

  • Keep the team within the CHW support role: ensure CHWs do not give clinical advice, bring clinical questions to the Consultant Nurse Practitioner, and route clinical concerns to the participant's care team.

  • Serve as the first point of escalation for safety and wellbeing concerns; support debriefing and secondary-trauma prevention with the Behavioral Health Consultant.

  • Participate in hiring and performance evaluations with the Director.

Complex cases

  • Take the lead on, or co-manage with the assigned CHW, cases that exceed a CHW's scope or experience — multiple chronic conditions, behavioral health needs, housing instability, immigration stress, or safety risks.

  • Work with the Consultant Nurse Practitioner on clinical escalation, and with clinical partners on shared participants and warm handoffs.

Direct service

  • Carry a reduced caseload, modeling the full CHW scope: intake, screening, navigation, home visits, closed-loop referrals, and follow-up.

  • Document every contact, referral, and outcome in Salesforce; protect participant privacy under HIPAA.

Program support

  • Track team performance against program targets in our database; flag gaps early and work with the Director on quality-improvement actions and funder reporting.

  • Contribute to workflows, protocols, and training materials; represent the CHW team at partner meetings.

Required Qualifications
  • Five or more years of experience as a Community Health Worker or in a comparable frontline community health role.

  • Maryland Department of Health (MDH) CHW certification, or eligible to certify within the onboarding period.

  • Demonstrated experience managing complex, high-need participants and coordinating with clinical and social-service partners.

  • Strong coaching, communication, and de-escalation skills; able to give clear, respectful feedback.

  • Comfortable with Salesforce or a similar system; strong documentation habits.

  • Reliable transportation and valid driver's license; flexible schedule, including occasional evenings or weekends.

Preferred Qualifications
  • Experience supervising, supporting, or mentoring frontline staff — formal or informal.

  • Speaks one or more languages in addition to English. Bilingual Spanish candidates are strongly encouraged to apply.

  • From or closely connected to the communities served.

  • Training in motivational interviewing, trauma-informed care, and/or Mental Health First Aid.

Physical & Working Conditions
  • Community-based role with regular travel across Frederick County, including home visits and partner sites.

  • Ability to lift light materials, navigate varied home and community environments, and work occasional evenings or weekends.
