As a Hospice Social Worker / Bereavement Coordinator, you will serve in a dual role supporting both hospice patients and families throughout the end-of-life journey and providing ongoing bereavement support following the loss of a loved one.
As a member of our interdisciplinary care team, you will provide comprehensive psychosocial support, counseling, resource coordination, advance care planning, grief support, and bereavement services. You will also help develop and coordinate programs that support families and caregivers before and after the death of a hospice patient.
We are looking for a compassionate, empathetic, dependable, and solutions-oriented professional who understands the complexities of grief, loss, and end-of-life care and is committed to providing dignified, person-centered support.
This is a community-based position requiring the ability to travel throughout the assigned service area. The role may also include participation in an on-call rotation as required.
What You'll DoResponsibilities may include, but are not limited to:
💙 Provide Comprehensive Psychosocial Support- Conduct thorough psychosocial assessments of hospice patients and their families within required timeframes.
- Assess patients' and families' coping mechanisms, support systems, strengths, preferences, and available resources.
- Identify psychosocial needs and develop individualized care plans to address those needs.
- Provide compassionate and culturally sensitive support to patients and families throughout the end-of-life journey.
- Advocate for patients' wishes, values, preferences, and autonomy throughout hospice care.
- Collaborate with the interdisciplinary team to address changing psychosocial needs and support continuity of care.
- Provide individual and family counseling related to grief, loss, anticipatory mourning, end-of-life decision-making, and other psychosocial concerns.
- Provide crisis intervention, stress management, coping skills, and emotional support as appropriate.
- Help patients and families navigate emotional, interpersonal, and practical challenges associated with serious illness and end-of-life care.
- Educate patients and families about the dying process and provide emotional preparation for anticipated loss.
- Make visits as scheduled and participate in the on-call rotation as required.
- Complete documentation and paperwork in accordance with hospice policies and procedures and the physician's Plan of Care for Medical Social Services.
- Facilitate conversations regarding advance care planning, healthcare directives, durable power of attorney for healthcare, and goals of care.
- Assist patients and families in making informed decisions regarding hospice enrollment, treatment options, and end-of-life care preferences.
- Support patients and families in navigating complex end-of-life decisions with compassion and respect.
- Ensure patients' wishes, values, and autonomy remain central to the care planning process.
- Connect patients and families with appropriate community resources and support services.
- Assist with referrals for financial assistance, caregiver support, home health services, support groups, bereavement counseling, and other community-based resources.
- Collaborate with interdisciplinary team members to coordinate referrals and support continuity of care.
- Develop knowledge of local community resources and services available to hospice patients and families.
- Help patients and families access resources that promote comfort, stability, and quality of life.
- Provide anticipatory grief support to patients and families facing the impending loss of a loved one.
- Educate patients and families about the grieving process and provide emotional preparation for bereavement.
- Conduct bereavement assessments to identify the needs, coping mechanisms, support systems, and resources of bereaved family members and caregivers.
- Provide individual counseling and ongoing follow-up support to bereaved family members and caregivers.
- Offer crisis intervention and additional support during periods of heightened grief or emotional distress.
- Provide referrals to appropriate grief counseling, support groups, community resources, and other services as needed.
- Facilitate grief support groups, workshops, educational sessions, and other bereavement programs.
- Coordinate scheduling, logistics, and content for support group meetings and educational programs.
- Create a safe, welcoming, and supportive environment for individuals and families experiencing loss.
- Develop resources and educational materials that promote healthy coping and understanding of the grieving process.
- Support memorial services, remembrance ceremonies, and other programs honoring hospice patients and their lives.
- Work with families and team members to create meaningful memorial tributes and remembrance activities.
- Build relationships with community organizations, religious institutions, healthcare providers, and other community partners.
- Participate in community outreach and educational activities related to hospice, grief, loss, and bereavement.
- Provide education and training to hospice staff and volunteers regarding grief, loss, bereavement support, and communication with grieving individuals.
- Help increase awareness of available bereavement services and community resources.
- Represent Cadre Hospice professionally and compassionately within the communities we serve.
- Complete all required documentation, assessments, care plans, visit notes, and bereavement records in accordance with hospice policies and procedures.
- Follow the physician's Plan of Care for Medical Social Services.
- Maintain accurate, timely, and compliant documentation.
- Communicate changes in psychosocial and bereavement needs to the interdisciplinary team.
- Collaborate with physicians, nurses, chaplains, aides, volunteers, and other team members to support comprehensive patient and family care.
- Perform other duties as assigned by leadership.
The ideal candidate will demonstrate:
- Master's degree in Social Work (MSW) from an accredited program. (Required)
- Minimum of one (1) year of experience in social work, counseling, bereavement, or a related healthcare field.
- Current automobile insurance and a valid driver's license.
- Reliable transportation and the ability to travel throughout the assigned service area.
- Strong understanding of grief and loss, psychosocial dynamics, family systems, and bereavement.
- Knowledge of grief counseling, bereavement interventions, and trauma-informed care principles.
- Excellent communication, interpersonal, and counseling skills.
- Ability to establish rapport and build trust with patients, families, and bereaved individuals.
- Strong organizational skills and attention to detail.
- Ability to work independently while also collaborating effectively as part of an interdisciplinary team.
- Commitment to the principles of hospice and palliative care.
- Compassionate, patient-first mindset with a commitment to dignified, person-centered care.
- LCSW or LMSW licensure in the state of practice.
- Previous hospice or palliative care experience.
- Previous experience providing grief counseling, bereavement support, or end-of-life counseling.
- Experience facilitating grief support groups, workshops, or educational programs.
- Experience coordinating memorial services or remembrance programs.
- Knowledge of local community resources and support services.
- Experience providing education or training related to grief, loss, hospice, or bereavement.
- Experience working with interdisciplinary healthcare teams.
- Strong understanding of the unique needs of individuals and families experiencing loss.
