Job Title: PRN Hospice Rabbi

Coverage Area: Northbrook, Buffalo Grove, Des Plaines, Glenview, Elk Grove Village, and surrounding areas.

Schedule: PRN-As needed/ Pay Per Hour

Job Summary: Primary function is to serve as the Jewish spiritual care specialist, addressing the spiritual needs of hospice and palliative care patients in their place of residence. Conducts spiritual assessments, counseling, religious support, and referrals to meet the needs of patients and their families while honoring Jewish traditions, beliefs, and practices.

Job Qualifications:

Education: Ordination as a Rabbi from an accredited Rabbinical Seminary or equivalent recognized religious institution.

Licensure: Per state guidelines, if any. Current driver's license.

Experience: One year of pastoral care, spiritual counseling, congregational leadership, or healthcare-related spiritual care experience preferred.

Skills: Working knowledge of Jewish traditions, community resources, and end-of-life practices. Good interpersonal and communication skills.

Transportation: Reliable transportation and valid auto liability insurance.

Environmental and Working Conditions:

Works in patients' homes, nursing facilities, assisted living facilities, and other care settings. Occasional exposure to infectious diseases; ability to travel locally; some exposure to unpleasant weather.

Essential Functions:

  • Assesses hospice or palliative care patients to identify spiritual needs and contributes to the care plan as evidenced by documentation in the clinical record and IDG/IDT conference minutes.
  • Provides Jewish spiritual counseling, prayer, religious rituals, blessings, and guidance to patients and families.
  • Attends IDG/IDT meetings and participates in the patient care planning process.
  • Uses effective interpersonal relations and communication skills to achieve desirable patient and family outcomes.
  • Collaborates with the IDG/IDT to ensure coordination and continuity of care.
  • Maintains accurate and timely documentation of care provided using EMR.
  • Communicates changes in patient or family spiritual needs to the care team.
  • Provides emotional support, grief support, and guidance to patients, families, and caregivers.
  • Assists with Jewish end-of-life traditions and religious observances as requested by patients and families.
  • Ensures that all interventions adhere to hospice philosophy, policies, and regulatory guidelines.
  • Applies infection control and standard precautions while providing patient care.
  • Participates in hospice-sponsored education and professional development activities.
  • Promotes the Hospice philosophy and administrative policies.
  • Provides effective communication to patients, families, team members, and other healthcare professionals.

Requirements:

Physical and Mental Effort:

Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients. Requires working under stressful conditions to meet deadlines, patient needs, and referral coordination demands. Requires strong organizational skills, critical thinking, communication abilities, and the ability to make timely clinical decisions.
