REHAB SOCIAL WORKER - LSW

Position Summary

The primary purpose of a Licensed Social Worker (LSW) working in the Woodlawn building is to assist with the psycho social issues which impact the discharge planning, through assistance and counseling of the patients and families, including services and resources available to meet the patient's needs at time of patient's transition. The LSW works with the Interdisciplinary team in determining the patient's needs at time of transition. All services provided by LSW are in accordance with the Ohio Counselor, Social Worker and Marriage and Family Therapy Board.

Department Summary

Woodlawn Inpatient Rehab is a 60 bed inpatient rehabilitation facility.

Qualifications

  • Current license in the state of Ohio as a social worker.
  • Graduate of an accredited school of social work or related degree with eligibility for licensure in the state of Ohio.
  • Demonstrate commitment to the delivery of ethical social work practice as defined by the NASW code of ethics.
    • e.g. Prior LSW experience preferred

  • Prior post-acute experience preferred (inpatient rehab, skilled nursing or home care).

Responsibilities & Expectations

  • Completes a face to face assessment with the patient within 4 days of admission Assists with the notification of families on the team meeting process for residents Completes phone call to family with in the first 2 days for baseline ADL data, discharge expectations and documents in the assessment.
  • Investigates the desired plan for discharge and communicates plan to all team members.
  • Attends team prepared to discuss needs and barriers to discharge plan.

  • Makes follow up calls to the family when absent from team meeting Assists with meeting the psycho-social needs for the patients.
  • Meets with patients and families to assist with meeting discharge needs Assists with obtaining resources for the discharge plan.
  • Communicates changes in the plan to the patient, families and team as they become known Advocates for the patients' and families' needs and wishes.
  • Assists in development of Advanced Directives.
  • Assists with the completion of Medicaid applications and follow up Assists with insurance verification for next level as needed.
  • Assists with obtaining prior authorizations for services during and post discharge as they are known.
  • Completes the exit interview with the patients and families to assure agreement with de plan.
  • Communicates timely with other team members and receiving levels of care.
  • Delegates appropriately with social service designee to assure safe discharge plan and timely completion of tasks.
  • Communicates with outside organizations
  • Assists with the communication of Non Coverage letters for all payers/Important Medicare Care Notice as directed.
  • Assists the transfer of Medicare/Insurance covered stay to private pay stay on Woodlawn campus.

Working Conditions

  • Works Monday through Friday with potential to work on weekends 8am to 4:30pm.
  • Weekend on call rotation.
  • Lunch and other breaks are coordinated with other staff to maintain adequate coverage.
  • Subject to frequent interruptions during the performance of duties.
  • Performing patient care responsibilities as required.

Hazardous Exposure Category

Category III - Includes tasks that involve no exposure to blood, body fluids, or tissues and Category I tasks are not a condition of employment. The normal work routine involves no exposure to blood; body fluids or tissues (although situations can be imagined or hypothesized under which anyone anywhere might encounter potential exposure to body fluids.) Persons who perform these duties are not called upon as part of their employment to perform or assist in emergency care or first aid or to be potentially exposed in some other way.
