About the Role:

As an Outreach Coordinator, you will serve as a bridge between the community and essential behavioral health services. This role is highly community-focused and allows you to meet individuals where they are-whether in shelters, on the street, or in other community settings-and help guide them toward care and resources.

You'll work both independently and alongside a multidisciplinary team to assess needs, remove barriers, and connect individuals to housing, treatment, and supportive services. This is a dynamic, purpose-driven role ideal for someone who is passionate about advocacy, relationship-building, and making a lasting impact in the lives of others.

What You'll Do

  • Conduct outreach in the community (shelters, clinics, and other settings) to engage individuals experiencing homelessness
  • Assess needs and connect individuals to appropriate behavioral health and support services
  • Collaborate with a multidisciplinary team and community partners to coordinate care
  • Serve as an advocate, helping clients navigate systems and access housing, employment, and treatment resources
  • Build and maintain relationships with referral sources, community agencies, and local stakeholders
  • Develop and track service plans with clients, supporting progress toward stability and self-sufficiency
  • Maintain accurate and timely documentation in client records and the EMR
  • Participate in outreach events and community engagement efforts to expand resources and awareness

What We're Looking For

  • High School Diploma required; Associate's degree preferred
  • Experience working with individuals experiencing homelessness, mental health, or substance use challenges preferred
  • Strong interpersonal and communication skills with a passion for helping others
  • Ability to work independently in community-based settings while collaborating with a team
  • Knowledge of community resources and service systems is a plus
  • Valid Florida Driver's License with acceptable driving record
  • Must be at least 21 years of age and meet company insurance requirements
  • Ability to travel locally to conduct outreach and attend community events
