Colfax, WA

Schedule: Friday, Saturday, and Sunday 0600-1830 (12-hour shifts) Pay: $40 - $50/hour

Position Summary:

Under the direction of a Registered Nurse (RN), the LPN provides resident-focused nursing care and treatment to elderly, acutely ill, and injured residents. This role combines hands-on clinical skills with compassionate care to support resident health, comfort, dignity, and quality of life. The LPN works closely with residents, families, physicians, and interdisciplinary team members to promote wellness, prevent illness and injury, and ensure the highest standards of care.

What You'll Do:

  • Deliver exceptional nursing care to residents/patients with compassion and professionalism.
  • Provide direct nursing care within the scope of practice and established competencies.
  • Initiate and communicate resident concerns using the SBAR process and collaborate with the RN on assessments and recommendations.
  • Assist with physician rounds and interdisciplinary care activities.
  • Administer treatments and monitor resident responses to care.
  • Participate in resident assessments, evaluations, and care planning.
  • Review and update care plans as needed in accordance with state regulations.
  • Communicate resident condition changes to healthcare team members and families.
  • Provide guidance and oversight to Medication Aides/Technicians during medication and treatment administration.
  • Administer medications accurately and safely as prescribed.
  • Monitor, document, and report medication effectiveness and adverse reactions.
  • Maintain accurate narcotic records and medication documentation.
  • Coordinate medication ordering, prior authorizations, and pharmacy communications.
  • Ensure medication carts and treatment supplies remain organized, stocked, and compliant.
  • Maintain timely, accurate, and compliant clinical records.
  • Review, transcribe, and implement physician orders in accordance with state regulations.
  • Communicate effectively with physicians regarding resident needs and changes in condition.
  • Assist with admissions, discharges, and clinical transitions of care.
  • Act quickly and effectively during medical emergencies.
  • Provide CPR and other emergency interventions within scope of practice.
  • Support resident safety through careful observation and prompt clinical decision-making.

Requirements:

Education

Graduate of accredited LPN School of Nursing.

Licenses/Certification

Current valid LPN license in the state employed.

Valid CPR certification.

Experience

Six months experience in a long-term care environment preferred.

state’s specific background check requirements prior to contact with patients/residents.
