Medical Residential LPN
Responsible for the health and welfare of the people supported in the medial residential group home and providing a family type home atmosphere. Manages all activities including upkeep of home and food budgeting. Acts as a part of a team and fills in the gaps in emergency situations and assists in other group homes when the need arises.
Reports to: Agency RN for clinical and Residential Directors and Residential Coordinators for residential
Supervises: None
Works with: Individuals we serve, human resources, case managers, clinic staff, residential staff, employment staff
External Relationships: TNDIDD, Managed Care Organizations, family/guardians/conservators, employers, community partners, Independent Support Coordinators, Support Coordinators (ECF), physicians, pharmacy
Qualifications: Current unencumbered LPN license
Work Experience: Previous experience with ID/DD population preferred but not required. Time management skills and ability to multi-task are beneficial in this position
Physical Requirements: Sight and hearing, ability to lift/assist people supported and to intervene during behavioral episodes in a manor in keeping with company policies
Responsibilities:
1. Ensure health and safety of people supported by ensuring adequate exercise, providing assigned nursing services, maintaining up to date postings of medical/emergency information, ensuring home is hazard free, participating routinely in training for fire evacuation, maintaining clean/sanitary environment and reporting any maintenance needs promptly.
2. Ensure a family atmosphere through interaction, counseling, outings, conversation, etc.
3. Meet the needs of the people supported through training and maintain a clean home, meal preparation, assisting with personal hygiene, and shopping.
4. Work on training actions and BSP actions/objectives (when indicated) as outlined in program plans with people supported on a regular basis. Document as specified. Meet monthly with the case manager on these and other issues.
Participate in monthly house meetings.
5. Encourage people supported to follow dietary menus. Post all menus/special diets for easy access for other staff.
6. Plan and follow through with regularly scheduled recreational activities at the house.
7. Work regular schedule, participate as a team player, offer to cover, or swap shifts when time off is needed. Sertoma nurses are required to cover shifts with existing nursing staff.
8. Distribute medications and other medical treatments as prescribed. Maintain daily counts and reorder medications as necessary according to agency policy and procedures. Report medication errors when discovered.
9. Distribute spending money, help residents cash paychecks and counsel and assist them with financial/spending skills.
10. Attend circle of support meetings, monthly staff meetings and in-service meetings, ISP meetings. Provide input to the RN, case manager and residential department on all plans.
11. Maintain the vehicle logs to include regular cleaning, monthly mileage form, gas receipts etc. Promptly report any vehicle problems to the maintenance department via work order or phone call to maintenance department.
12. Familiarize self with licensure, fire marshal and quality assurance standards and always maintain group home within these standards. Meet with residential director or residential coordinator regarding any issues.
13. Work closely with all pertinent staff as a team member to assure the highest standards and best quality of life for the people supported.
14. Communicate with the appropriate staff immediately regarding any concerns, problems, or outside issues from external relationships.
15. Complete and document nursing notes daily.

4. You will adhere to an acceptable level of professionalism to ensure the maintenance of confidentiality of discretion necessary in the maintenance of person supported and personnel records.
5. You will observe all Sertoma routine with regard to work schedules, presence during working hours which confirms to those of Sertoma, designated periods of break time, lunch hour, etc.
6. You will strive to perform all work assignments and activities in an acceptable manner.
7. You will perform any other duties as assigned by the Registered Nurse, Chief Operating Officer, Director of Residential Services, or Residential Coordinator.
8. Your regular attendance is a job requirement and more than one unplanned absence in a two-month period is considered excessive.
9. You must establish and maintain good working relationships with coworkers and people supported.
10. Rotating schedule with alternating weekends will be required.
